    • LAST NAME AGREEMENT

    • Before filling out this section, please check your Czech marriage certificate and confirm whether it includes a surname agreement:

      – If the surname agreement is included, pay attention to the section A only.
      – If the surname agreement is not included, skip the section A and fill out section B instead.

      Please complete only the fields that apply to your case and leave the others blank.

      ⚠ After your final PDF is generated, make sure to place your signatures under the section that has been filled out in the 'Dohoda o příjmení' area.

      If your situation is unclear, the agreement about last names can be completed during your consular appointment.

      Following section is for cases where one spouse wishes to use a hyphenated last name, combining their original surname with the shared surname agreed upon in the marriage. The original surname will be placed before the shared surname.

      For example:
      If the shared surname is Novák and the bride wishes to keep her original surname Černá, she may choose to use the surname Černá-Novák after marriage.

      This option applies to only one spouse and is not required.

    • Will one of you be adding their surname before the shared last name, as allowed by Czech naming rules?
    • REMOVING DIACRITICS / Surname as Stated in Foreign Marriage Certificate

      Use this section to enter surnames as they appear on your foreign marriage certificate, with the option to remove diacritics (e.g., Dvořák → Dvorak).

      Note: The husband and wife may each choose different surnames. The child will carry the surname of one of the spouses.
