By submitting this form, I hereby grant Chic Celebrations, LLC permission to use my ("guests") likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. Chic Celebrations reserves the right to refuse service upon arrival at the venue if certain contract agreements are not met. Additionally, any damages incurred to equipment during the event will result in the renter being charged to replace the damaged items.
