  • Safeguarding

  • 1. Are you representing an organisation that works with children, young people or adults at risk of harm?*
  • 2. Does your organisation have a written policy that demonstrates the organisation’s commitment to safeguarding children, young people or adults at risk of harm, and is it is available to all stakeholders (such as parents / carers, volunteers, etc.,)?

  • 6. Has your organisation clearly defined procedures for raising awareness of, responding to, recording and reporting concerns about actual or suspected incidents of abuse or harm? For example, Designated Safeguarding Officers/Leaders, a procedure to deal with concerns or incidents, how to assess concerns or incidents, confidential records of concerns / incidents
  • 7. Does your organisation have a written Code of Conduct/Behaviour which outlines the behaviour expected of all involved in the organisation? For example, guidelines on how to safely lead activities for children, young people and adults at risk of harm
  • 8. Has your organisation processes in place to share relevant information appropriately and safely with parents, carers, children, young people, staff, volunteers, and other agencies, including information on who the designated safeguarding officer/leader is and how to contact them? For example, data protected emergency contact information, confidential communication forums, etc.
  • 9. Does your organisation adhere to guidelines that ensure the general safety and effective management of activities? For example, ensuring participants depart an activity with a permitted adult and there is a recommended number of leaders in relation to the number of participants.

  • Youth Bookings (17 & under)

  • Those who instruct, coach and/or manage teams or individuals aged 17 and under must be appropriately trained and qualified, as follows:

    • Appropriate coaching qualifications for the level / age group
    • Access NI checked
    • Safeguarding trained
    • First Aid qualified

  • Requests for credit may be accepted on the understanding that a monthly VAT Invoice will be prepared on the basis of the original booking.  

     

    Credit Terms are strictly 30 days – if payment in full is not received within 30 days, all credit facilities will cease immediately.

  • Please confirm that the information provided is complete and accurate and acknowledge that Fermanagh & Omagh District Council may terminate any bookings, without notice, if it is discovered that you have provided incomplete, inaccurate or false information, or you fail to notify the Council about any change in your circumstances which may have the potential to impact any polices, insurance cover and/or accreditations/certifications which you hold and are relevant to the delivery of the service.

The information you provide on this form will only be used for the purpose of Grass Pitch and Changing Room bookings and will not be shared with any third party unless law or regulation compels such a disclosure.
