• The purpose of this questionnaire is to collect all relevant information we need to complete your personal tax return for the tax year ended 5th April 2025.

    Whilst some of the questions in the form will not be relevant to you, they serve as a prompt and for us to gain formal confirmation as to whether each type of income is relevant to you.

    A few tips from us before you start the questionnaire (as this has changed slightly from previous years):

    • You need to have all supporting documents to hand for each income type, e.g. P45, P60, P11D, interest statements etc. ready to upload.
    • Whilst you are able to scroll between sections within the form without losing any of the information you have entered/uploaded, you will lose all the data you have entered if you close or refresh this page.
    • Please include N/A for any mandatory fields not applicable to you. 
    • At the end of the form, you will be asked to declare that all information is accurate and complete and therefore please do not provide estimates or omit information.
    • This questionnaire covers the period 6th April 2024 to 5th April 2025.
