2025 TAMIAMI BASKETBALL SUMMER CAMP

GENERAL RELEASE: I have enrolled my child to participate in organized basketball and related athletic events, activities, and instruction given and conducted by Tamiami Youth Basketball, Inc. I am fully aware and recognize that the activities my child will engage in during his or her enrollment and participation in the foregoing activities will involve strenuous physical activity including, but not limited to, muscle strength, cardiovascular training, and athletic movements and activities that may pose a serious risk of injury to my child. For and in consideration of Tamiami Youth Basketball, Inc., permitting my child to enroll and participate in organized basketball and related athletic events, activities, and instruction given and conducted by Tamiami Youth Basketball, Inc., I hereby voluntarily release, discharge, waive, and relinquish any and all claims, demands, damages, actions, causes of action, or suits in equity of whatever kind or nature for personal injury, death, or property damage, occurring to me or my child as a result of engaging in such activities or receiving instruction given or conducted by Tamiami Youth Basketball Basketball Inc., or in any activities incidental to such activities or instruction, wherever or however any such injury, death, or property damage may occur, whether caused by the negligence of Tamiami Youth Basketball, Inc., or otherwise, and for whatever period during which such activities or instruction may continue. I also assume full responsibility on behalf of my child and myself for all risk of bodily injury, death, or property damage caused by the negligence of Tamiami Youth Basketball, Inc., or otherwise while engaged in any activity or instruction with Tamiami Youth Basketball, Inc. I also do, for myself and my child, and our heirs, executors, administrators, and assigns, release, waive, discharge, and relinquish any actions or causes of action for personal injury, death, or property damage that may arise from participating in organized basketball and related athletic events, activities, and instruction given or conducted by Tamiami Youth Basketball, Inc., and agree that under no circumstances will my or my child’s heirs, executors, administrators, and assigns prosecute or present any claim for personal injury, death, or property damage against Tamiami Youth Basketball, Inc., or any of Tamiami Youth Basketball, Inc. 's officers, agents, servants, or employees for personal injury, death, or property damage arising from participating in any such activities or instruction conducted by Tamiami Youth Basketball, Inc., whether such personal injury, death, or property damage be caused by the negligence of Tamiami Youth Basketball, Inc., or otherwise. I am aware that photographs or video may be taken of myself and my child by Tamiami Youth Basketball, Inc.’s staff, representatives, and agents, or third party professional photographers, news media, or volunteers during the organized basketball and related athletic events, activities, and instruction given or conducted by Tamiami Youth Basketball Basketball, Inc. I hereby waive the right to see or approve any publications of the photographs or video footage that are taken of myself and my child. I further authorize Tamiami Youth Basketball, Inc., to use photographs or videos of my likeness or my child’s likeness taken at Tamiami Youth Basketball, Inc., events, activities, or instruction for publication in any promotional or marketing materials, social media websites, advertisements, or television or radio coverage, without compensation to myself or my child.I hereby release and agree to hold Tamiami Youth Basketball, Inc from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Tamiami Youth Basketball, Inc. I understand that this release discharges Tamiami Youth Basketball, Inc from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Tamiami Basketball, Inc. This liability waiver and release extends to the program together with all owners, partners, and employees. I acknowledge that I have read the terms and conditions of this General Release and understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. My completion of the registration process attests to this on behalf of myself and my child, our heirs, executors, administrators, and assignees. I acknowledge that NO REFUNDS will be given unless an accepted doctor's note is provided.
