Permission, Medical Treatment Authorization, Release and Waiver, Indemnity & Hold Harmless Agreement

I (We) give permission for my child(ren) to participate in any Canyon Creek Presbyterian Church (CCPC) program or event, including any Nursery, Early Childhood, Elementary, Sacred Play, 5th and 6th grade, Middle School, High School, or other Children or Youth program or event, including VBS. These programs may include field trips, swimming outings, mission trips or retreats, and can also include transportation to and from such events, whether in the CCPC staff vehicles, private volunteer driven cars, buses leased by CCPC from professional transportation companies, air travel, or other transportation means. I (we) understand that this grant of permission is not specific to one event but shall include all events ("the events") in which my child participates under the auspices of Canyon Creek Presbyterian Church Children & Youth Ministries. This waiver must be signed whenever a new child enters a Canyon Creek Presbyterian ministry.

Medical Treatment Authorization

In case of injury to my child(ren), whether during participation in the events, transportation to or from the events, or otherwise, I hereby authorize the healthcare professional selected by CCPC to provide whatever medical treatment(s) he/she deems necessary to my child. I agree to make a claim for any medical expenses thereby incurred on my (our) personal insurance.

Indemnity and Hold Harmless Agreement

In consideration for Canyon Creek Presbyterian Church's organization and sponsorship of the event and allowing me or my child(ren) to participate in the event, I hereby agree to unconditionally release and agree to protect, indemnify and hold harmless, Canyon Creek Presbyterian Church, its managing representatives, members, employees, agents, successors, assigns, and other representatives (collectively, the "Released Parties") from and against any and all damages, injuries, illnesses, accidents, claims, demands, causes of action and damages to or loss of property, including  attorney's fees, resulting from or relating to my, or my child's, participation in the event whether or not caused in whole or in party by the negligence of any one or more of the released parties.

I hereby further conditionally covenant and agree that I, my spouse, legal guardians, heirs, estate, legal representatives, successors and assigns will not make any claim or institute any suit or action at law or in equity against any of the Released Parties that is based upon any injury, illness, accident, incident, occurrence or damages to or loss of property arising out of, incidental to or in any way resulting from my, or my child's, participation in the event whether or not caused in whole or in party by the negligence of any one or more of the released parties.

I acknowledge that I have read, understand and fully accept the terms and conditions of this Release and Waiver and unconditionally and voluntarily accept them. I agree that the laws of the State of Texas shall apply to this agreement and that any suit about this agreement or any underlying claim covered by this agreement must be filed in the State of Texas.

This Agreement covers all events, programs, ministries and related activities of CCPC that my children participate in during the one year period that begins with my submission of this Agreement during the provided time-frame (beginning during the summer Any participant that joins CCPC ministry at a later date is required to sign this waiver and release before participating.

BY DIGITALLY SIGNING BELOW, I ACKNOWLEDGE, AFFIRM, AND AGREE THAT I HAVE READ, UNDERSTAND AND ACCEPT ALL OF THE PROVISIONS OF THE AGREEMENT SET. I INTEND FOR MY DIGITAL SIGNATURE TO HAVE THE SAME LEGAL EFFECT AS IF I SIGNED A PAPER VERSION OF THE AGREEMENT ABOVE IN THE PRESENCE OF A MEMBER OF THE STAFF OF CANYON CREEK PRESBYTERIAN CHURCH; AND I INTEND BY MY SELECTING "SUBMIT" TO BIND MYSELF LEGALLY TO THE TERMS OF THE AGREEMENT.

CANYON CREEK PRESBYTERIAN CHURCH MEDIA AGREEMENT

I hereby consent that my child's name, image, and likeness, in media, photographs, film and/or electronic images and/or audio recordings of their voice may be used by Canyon Creek Presbyterian Church, its assigns or successors; furthermore, I hereby consent that such photographs, films, media, recordings, electronic images, and the plates, tapes and/or software from which they are made shall be their sole property, and they shall have the right to use these for the promotion within and without the church, as they may desire free and clear of any claim whatsoever on my part.

By signing below I acknowldge that I have read the above and agree to these terms and grant permission herein.
