    * By submitting this form, I acknowledge that participation in dance classes involves physical activity and I hereby release the dance studio and its instructors from any liability for injuries or accidents that may occur during classes or events.

    * We will only allow online bookings and reservation. No walk-ins are allowed. You may settle your booking up to 3 hours before the class schedule. Bookings and payments are non-refundable. All payments are final. You are allowed to cancel or reschedule up to 3 hours before, otherwise, payment will be forfeited.
