In the sections below, any reference to the singluar includes BOTH parents and/or any other representatives of the family.

Once you have completed this, download the PDF copy you will receive in your email, and upload it to your registration in the TS Documents section (exactly the same way you upload the Medical Form). You only need to complete this form ONCE for all players in your family - simply upload the same form for all players.

ACCEPTANCE OF THE CMHA VOLUNTEER POLICY IS REQUIRED FOR YOUR PLAYER(S)' REGISTRATION TO BE CONFIRMED.

(U21 players are not subject to the policy but will need to upload a form to the registration to be able to proceed).

A NOTE REGARDING ON-ICE HELPERS ON TEAMS

BY CLICKING PAST THIS PAGE, YOU HAVE BEEN MADE EXPLICITLY AWARE THAT:
- ON-ICE HELPERS ARE HCR-ROSTERED TEAM OFFICIALS EVEN THOUGH THEY CANNOT GO ON THE BENCH DURING GAMES.
- ON-ICE HELPERS MUST BE CERTIFIED WITH A CURRENT CRIMINAL RECORD CHECK AND RESPECT IN SPORT FOR ACTIVITY LEADERS AND ROSTERED TO THE TEAM IN THE HCR.
- FAILURE TO BE ROSTERED IN THE HCR AS AN ON-ICE HELPER MEANS THAT YOU WILL NOT RECEIVE ASSOCIATION VOLUNTEERING CREDIT, EVEN IF YOU WERE HELPING THE TEAM ALL SEASON ON THE ICE DURING PRACTICES.
- IF YOU ARE PLANNING TO VOLUNTEER ON YOUR CHILD'S TEAM AS AN ON-ICE HELPER, PLEASE ENSURE YOU ARE COMPLYING WITH REQUIREMENTS.
