In the sections below, any reference to the singluar includes BOTH parents and/or any other representatives of the family.

Once you have completed this, download the PDF copy you will receive in your email, and upload it to your registration in the TS Documents section (exactly the same way you upload the Medical Form). You only need to complete this form ONCE for all players in your family - simply upload the same form for all players.

ACCEPTANCE OF THE CMHA VOLUNTEER POLICY IS REQUIRED FOR YOUR PLAYER(S)' REGISTRATION TO BE CONFIRMED.

(U21 players are not subject to the policy but will need to upload a form to the registration to be able to proceed).

Acknowledgement of Volunteer Policies

The list below is not exhaustive. These are just the most commonly misunderstood parts of the overall policy. If a part is not included here, that does NOT mean you have not accepted the whole policy generally by signing this document. By signing this document, you accept ALL aspects of the policy whether it is referred to below or not. The policy enforces association volunteering. Any reference to volunteering below refers to association volunteering, not team or tournament volunteering.
