Crown HER Mentoring New Mentee Interest Form

Thank you for your interest in Crown HER Mentoring! We are excited to welcome young girls who are ready to grow, lead, and make a difference. Please review the eligibility requirements below and, if you’re still interested, have your child complete the following questions.

Eligibility Requirements

✔️ Ages 09-19 and committed to leadership and personal development.

✔️ Maintain an 80% overall participation rate in program activities.

✔️ Maintain a B average or higher in school (academic recovery options available if needed).

✔️ Attend Mentor Meetings , every other Sunday (3:00 PM-6:00 PM) at the Crown HER Empowerment HUB.

✔️ Complete at least 5 hours of community service per quarter.

✔️ Actively participate in fundraisers, workshops, and events.

✔️ Demonstrate respect, responsibility, and a positive attitude while upholding Crown HER Mentoring values.

✔️ Membership dues apply. $150/year . Siblings Discount Available.

** MUST HAVE TRANSPORTATION

Once this form is submitted, it will be reviewed by our leadership team, and you will be contacted for an interview if selected to move forward in the process.
