NBW Adult Basketball Player Waiver and Release of Liability

I acknowledge that participation in the Next Basket Wins LLC Adult Men's Basketball League (hereinafter referred to as the League) involves inherent risks, including but not limited to bodily injury, disability, paralysis, or even death. I understand that basketball is a physical sport that may result in contact injuries, falls, or other accidents.

I voluntarily choose to participate in the League and assume all risks associated with my participation, including but not limited to risks arising from:

-Collisions with other players, walls, equipment, or flooring.

-Physical strain, exertion, or pre-existing medical conditions.

-Negligence of other participants, referees, or League staff.

-Physical altercations and arguments on-site or in adjacent parking lots during and after a game

-Any other unforeseen risks related to the game of basketball.

3. Release of Liability and Waiver

In consideration of being allowed to participate in the League, I hereby release, waive, and discharge Next Basket Wins LLC, its owners, directors, officers, employees, agents, sponsors, facility providers, and any affiliated entities (collectively, the Released Parties) from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, death, loss, or damage that may occur as a result of my participation, whether caused by the negligence of the Released Parties or otherwise.

I agree to indemnify, defend, and hold harmless the Released Parties from any claims, lawsuits, damages, or expenses (including attorneys fees) arising from my actions or participation in the League.

5. Medical Treatment and Insurance

I certify that I am physically fit to participate in the League. I understand that Next Basket Wins LLC does not provide medical insurance, and I am solely responsible for any medical expenses incurred as a result of my participation. I authorize League representatives to seek emergency medical care on my behalf if necessary.

I agree to abide by all League rules and regulations, including good sportsmanship, respect for other players and officials, and compliance with facility policies. I understand that failure to adhere to these rules may result in suspension or expulsion from the League without refund.

I grant Next Basket Wins LLC permission to use photographs, videos, or other media of me taken during League activities for promotional or marketing purposes without compensation. The use of this video and content can and will be shared on social media platforms including but not limited to Instagram, Facebook, Tik Tok, and Youtube and could appear on the Next Basket Wins website.

8. Acknowledgment of Understanding

I have read and fully understand this waiver and release of liability. I acknowledge that by electronically executing this document below, I am waiving certain legal rights. I filled out this form and agree voluntarily and with full knowledge of its significance.
