Participant Agreement & Release – Interactive Dream Therapy Workshop
1. Consent to Record
I understand that this Interactive Dream Therapy (IDT) workshop may be audio and/or video recorded for educational, promotional, and commercial purposes. I consent to being recorded during the workshop, including my voice, image, likeness, and any content I voluntarily share.
2. Use of Recordings
I grant Phoenix on Fire Healing Arts, Jazzie Smith, and their affiliates or assignees the perpetual, irrevocable right to use, reproduce, publish, edit, distribute, sell, or otherwise utilize these recordings—whether in whole or in part—for any lawful purpose, including but not limited to:
Online courses or workshops
Promotional and marketing materials
Books, podcasts, or other media
Paid or monetized platforms
3. Waiver of Compensation
I understand and agree that I will not receive financial compensation, royalties, or reimbursement for the use of these recordings, now or in the future.
4. Voluntary Participation
I understand my participation in this workshop is voluntary. If I prefer not to be recorded, I agree to notify the facilitator in advance and participate with my video and/or audio turned off. I understand that some aspects of the workshop may be limited or unavailable to me if I choose not to be recorded.
5. Release of Liability
I release and hold harmless Phoenix on Fire Healing Arts, Jazzie Smith, and any associated individuals or entities from any and all claims, demands, or causes of action related to the use of these recordings or my participation in the workshop.
6. Disclaimer and Personal Responsibility
I understand that Interactive Dream Therapy (IDT) is a personal development and inner exploration modality. It is not a substitute for licensed psychological counseling, medical advice, diagnosis, or treatment. I take full responsibility for my emotional, mental, and physical well-being during and after this experience.
7. Certification & Scope of Practice
Certification as an Interactive Dream Therapy Practitioner acknowledges that the participant has completed training in the IDT method developed by Jazzie Smith of Phoenix on Fire Healing Arts. This certification does not constitute a license to practice psychotherapy or medicine. I agree to use IDT within the legal and ethical boundaries of my jurisdiction and personal scope of practice.
8. Assumption of Risk & Personal Choice
I accept full responsibility for my choices, actions, and outcomes resulting from my participation in this workshop and any application of the IDT method thereafter. I release Jazzie Smith and Phoenix on Fire Healing Arts from all liability for harm, discomfort, or perceived negative effects. I understand that if I choose to facilitate IDT work outside of this workshop, I do so at my own discretion and risk.
