PERMANENT JEWELRY LIABILITY WAIVER AND RELEASE AGREEMENTE

This Agreement is between Gypsy Jewelry LLC ("Service Provider") and the undersigned Client. By signing, Client agrees to the terms and releases Gypsy Jewelry LLC from liability as outlined below.

Gypsy Jewelry LLC offers mobile permanent jewelry services, including welding and securing custom-fit jewelry.

I understand the services provided.

 •I understand that Permanent Jewelry is a welded, clasp-free piece that will need to be cut off If you choose to remove it

•Permanent jewelry involves tools (e.g., welders) that may pose risks like burns, cuts, or irritation.

•Jewelry may break, malfunction, or get caught on objects, causing injury. Gypsy Jewelry LLC is not Liable if this happens.

•I awknowledge that jewelry may need to be rewelded if it breaks due to normal wear and tear or a accident, and I understand that a re-welding fee may apply.

•Metals used may cause allergic reactions or skin irritation.

•Medical procedures may require removal, and removal costs are the Client's responsibility.

•Jewelry is not verified lead-free (though sourced from reputable wholesalers)

•If a child recieves permanent jewelry, the parent/guardian is responsible for removing it if child outgrows it. Gypsy Jewelry LLC is not liable for injuries related to outgrown jewelry.

 

I understand and accept these risks.

•Jewelry can be removed using nail clippers or scissors at the weld point (jump ring) and re-welded later.

•Jewelry is not indestructible and may break under excessive force for safety. 

•Client accepts responsibility for jewelry care and maintenance.

I understand how to remove and care for my jewelry.

The Client releases Gypsy Jewelry LLC from all claims related to:

•Injuries from the application, removal, or wearing of jewelry.
•Allergic reactions or irritation caused by materials.
•Loss, damage, or required removal of jewelry for medical purposes.

I release Gypsy Jewelry LLC from liability as outlined above.

Client confirms they are 18 years or older. If under 18, a legal guardian must sign this form.

•I confirm I am at least 18 years old OR my legal guardian is completing this form.

By signing below, the Client confirms they have read, understood, and agree to all terms in this Liability Waiver and Release Agreement.
