Insurance Form Request

We're very happy to support you with the completion of an insurance/special authority form. 

Please note:

• This form is designed to facilitate the completion of your insurance form/special authority request by your practitioner only.
• It is your responsibility to request the correct form from your insurance provider.
• Please follow the instructions to help us promptly complete your form request.
• Upon completion, a member of the Felix medical admin team will contact you with the completed form.
• It is your responsibility to submit the completed form to your insurance provider. 
• Submitting duplicate requests may lead to processing delays. 

By submitting your insurance request, you acknowledge that a third party will store the information provided. If you prefer, you may also submit your request through the Patient Support chat located in your Felix account or the bottom right corner of our Contact Us page.

To avoid delays, please follow these steps when submitting your form:

1. Request your form directly from your insurance provider

2. Ensure patient details section is filled out, signed and dated. 

3. Leave blank any sections intend to be completed by the Practitioner

This includes your medical and medication info. Your practitioner will use your Felix profile to complete these sections, so please ensure it’s up to date or contact them if updates are needed.

4. Ensure form is in PDF format.

Please note that delays may occur if forms are incorrectly submitted.

• Due to high volume, the current turnaround time for form completion is appoximately 3-5 business days
• Our medical admin team will be in touch once your form is complete, or if more information is required
• Please avoid submitting duplicate requests as these cause delays in processing your form
