Rummy's Beach Club New Client Form

You can bring up to 3 people and 3 dogs. Dogs must know each other.

Sorry, no children under 16 are allowed on the property for liability reasons.

Please have your vaccination records handy. You will need them to complete this form.

Cameras & video cameras are welcome and encouraged.  Rummy’s Beach Club is also happy to take pictures and or videos and share them with you on Facebook – there is no extra cost. Pictures taken by Rummy’s Beach Club are the property of Rummy’s Beach Club and will be used in social media, advertising, marketing, blogs, promotional materials, printed for sale or other reasons.

I hereby authorize Rummy’s Beach Club, LLC, hereafter referred to as “Rummy’s,” and its agents, employees, and volunteers to photograph, videotape, audio record, televise, duplicate, and/or otherwise record images of my animals and/or me and the likeness.  I irrevocably authorize Rummy’s to publish photographs taken (whether in the past or future) of my pets and/or myself for use in the Rummy’s company publications including those that are printed, published online, or created in video form. 

I attest that I have full authorization to consent to publication of these photos.  Further, I hereby release and hold harmless Rummy’s from any reasonable expectation of privacy or confidentiality associated with these images.

I understand that Rummy’s will own these images and recordings.  Rummy’s may take pictures of me and any of my dogs / pets.  Pictures taken by Rummy’s are the property of Rummy’s and will be used in social media, advertising, marketing, blogs, promotional materials, printed for sale or other reasons.  I understand that any pictures taken by Rummy’s are the property of Rummy’s Beach Club and they are allowed to use any pictures with me or my dogs for any purpose they choose. 

I waive any right to inspect or approve these recordings or material that may be used with them now or in the future, whether that use is known to me or not. 

I further acknowledge that my participation is 100% voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs (whether in the past or future) or participation in any company marketing materials or other Company publications.  I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.  I hereby release Rummy’s Beach Club, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation. 

I have read this RELEASE and understand all its terms. I execute it voluntarily and with full knowledge of its significance. This agreement has no time limit and is valid and enforceable for any and all future activity with my dog[s]. The information provided by me is complete and accurate and I agree to all its terms and conditions.

I release Rummy’s, its regents, employees, and agents from all liability arising out of the use of these recordings, including but not limited to any claims arising out of my right of privacy or right of publicity and any claims based on any distortions, optical illusions, of any mechanical reproductions.  I understand that I will not be compensated for any use of sale of these images/recordings.  I understand that this is a legal document and represent that I have read it and understand it and am willing to sign it voluntarily.

No alcoholic beverages.

Age 16 and up only – with proof of age (driver’s license, student ID, etc.).

DO NOT feed your dog 3 hours before and after swimming to help prevent regurgitation / defecation in the pool.

No food or glass in the pool area.

Cancellations without 24 hour notice incur charges. If weather is questionable, please call to confirm your appointment.

Dogs with vomiting, diarrhea, no appetite, etc. are not welcome -- if your dog is ill or has been near a dog with an infection, please do not bring him swimming for 7 days after exposure.

Aggressive or out of control dogs and people will be asked to take a time-out or leave.

We take the cleanliness of our beautiful pool seriously. We ask you to do the same.

Allow 5 days to elapse after using topical flea treatment. If you come swimming once weekly or more - topical flea preventative will probably not work - you will need a pill form of flea preventative.

Please brush your dog before swimming. Short & smooth dog nails are appreciated.

Dogs new to you must live and thrive in good health in your home for at least 14 days prior to swimming.

When you get to Rummy’s, the first 5-10 minutes will be spent letting your dog smell and go potty. Rummy’s is a stimulating environment for dogs – don’t be surprised if they defecate 2 or 3 times here. Dogs drink a lot of water while swimming. In addition, being in water is a natural diuretic - your dog will need to urinate several times for a few hours after each session.

If you suspect your dog may defecate in the pool – for example if they are older and don’t always have control over their bowels or if they have a spinal cord injury, you are still welcome to swim here, we will just make you the last appt of the day so the pool can be cleaned overnight. We take half time breaks to decrease the likelihood of your dog defecating in the pool, however, if your dog defecates in the pool more than one time, your appointments will need to be the last one of the day so the pool can be cleaned overnight.

I agree to hold harmless and indemnify Rummy’s Beach Club, LLC, and Lisa Goebel, the owner, as well as her agents, employees, volunteers, successors and assigns from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog while in or upon the premises or grounds or near any entrance thereto, and I personally assume all responsibility and liability for such claim.

I further agree to hold the aforementioned parties harmless from any claim of loss of this dog by disappearance, theft, death or otherwise, and from any claim of damage or injury to the dog, caused or alleged to be caused by the negligence of the parties aforementioned, or by the negligence of any other person or any other cause or causes.

I understand that any pictures taken by Rummy’s Beach Club is the property of Rummy’s Beach Club and that they are allowed to use any pictures with me or my dogs for any purpose they choose.

I agree that there are inherent risks to me associated with the use of Rummy’s Beach Club, LLC, facilities arising out of or associated with use and conditions, such as swimming, wet floors, multiple dogs and owners and dog behavior. In consideration for Rummy’s Beach Club, LLC granting me permission to use these facilities, I agree to release Rummy’s Beach Club, LLC from liability arising out of or associated with such use, and hereafter waive any and all claims which may arise out of or be associated with such permissive use of the Rummy’s Beach Club, LLC, facilities.

A handling fee of $40.00 will be charged on all returned checks. In the event it is necessary to initiate collection proceedings on the account, Client will be responsible for all attorney’s fees and costs of collection.
