Please note: Proxy will need to have ID ready when picking up lockboxes. If proxy does not have photo ID, we cannot release lockboxes. ORRA Member Services will email when lockboxes are ready to pick up. Please ensure your SentriLock app is valid and current. SentriLock cannot assign lockboxes to those without the SentriRE Key or suspended SentriLock members.
All information on this form must be filled in, including member signature to ensure proper processing.
The lockbox will need to be paid for at the time of pickup. Please note the lockboxes are $122.48 each and we do not take payment over the phone. The proxy will be responsible for paying for lockboxes being picked up. You must have an active Sentrikey in order to purchase a lockbox. Account setup needs to be confirmed prior to Proxy pick up.
