Please fill out this form with as much detail as possible. An incomplete request will not be answered or fulfilled. A JTF representative will attempt to contact you via phone, text, or email for further information regarding your request. We will hold items up to 14 days. If a recipient fails to meet for a scheduled pick up time, all items will be returned to the closet program. Otherwise, all items are always first-come, first-serve. JTF will also do our best to fill wait list requests in order received as donations come in. **Before taking time to request, please note that we DO NOT ship equipment (i.e. wheelchairs, strollers, shower chairs, etc).**

Joseph Thomas Foundation (JTF) Closet Program – Terms and Conditions

Please read the following terms and conditions carefully. Your signature indicates your understanding and agreement with each statement.

1. Donation-Based Program
I acknowledge that the JTF Closet Program is supplied entirely through donations. I understand that my request for equipment or supplies may not be fulfilled within a specific time frame, and fulfillment is at the sole discretion of JTF staff.

2. Condition of Equipment and Supplies
I understand that any equipment and/or supplies I receive from the JTF Closet Program are gently used and provided “as-is.” JTF makes no guarantees as to their condition or functionality.

3. Limitation of Liability
I acknowledge that the Joseph Thomas Foundation is not responsible for any missing parts or damage to the equipment or supplies provided. I also understand that JTF is not liable for any injuries or harm that may result from the use of such equipment or supplies. I release the Joseph Thomas Foundation and its representatives from any and all liability associated with the use of items received.

4. Use of Media (Photos/Videos)
I give permission to the Joseph Thomas Foundation to use my photo(s) and/or video(s) for promotional purposes including but not limited to advertising, social media, printed materials, and Thank You cards sent to donors.

5. Prohibited Resale of Donated Items
I understand that selling, reselling, or profiting in any way from medical equipment or supplies received through the JTF Closet Program is considered fraud. If I am found to be engaging in such activities, I will be permanently disqualified from receiving future assistance from JTF. Any financial contributions I have made will not be refunded and will be considered donations.
