Please note: the Urban League of San Diego County DOES NOT determine eligibility; the participating lenders and LISC-San Diego will determine eligibility.

    Who Can Apply?

    To qualify for this program, applicants must meet the following criteria:

    • Teacher Qualification: Must be a certified educator.
    • Residency & Employment: Must teach within the boundaries of San Diego City Council District 5.

  • Welcome to Urban League of San Diego County (ULSDC) – Housing & Financial Education Program.

    Before scheduling you for an appointment with one of our counselors, we need you to complete and submit this form. Below are brief descriptions of the information that MUST be completed before the counseling session appointment.

    • The “Homeownership Client Disclosure Form” provides information about ULSDC and the services we provide that align with our mission. In addition, because we are a HUD-certified non-profit organization, most of our funding is through grants disclosed in this form. Lastly, this form also outlines our role in assisting you in obtaining a resolution with your lender and your role as the borrower(s).
    • The “Homeownership Intake Packet” will provide us with your general information. We also use this to keep track of your counseling sessions.
    • “Homeownership Privacy Policy and Practices of ULSDC,” which discloses procedures concerning any personal information you provide us.
    • The “Homeownership Spending Plan” collects information on family/ individual expenditures, mortgage expenses, and household income. This worksheet helps both the household and lenders see spending habits, recognize areas for change, and identify surpluses or deficits after all the expenditures have been accounted for.

  • The federal government requests the following information for certain types of loan applications and other programs to monitor compliance with equal credit opportunity, federal civil rights laws, fair housing and home mortgage disclosure laws, and our statistical monitoring. You are not required to furnish this information but are encouraged to do so. By providing this information, you will assist us in assuring that this program is administered in a nondiscriminatory manner. All answers are kept strictly confidential.

  • Urban League of San Diego County collects personal information about the people we serve in a computer system CounselorMAX (CMAX). We collect and maintain client information in a private and secure computer system. People who work for ULSDC will use this information to help provide and track services for you or your family. This information may be shared with auditors or others who have legal rights to review the work of this agency, including private and public funders. All employees using the CMAX system must sign an agreement to protect your privacy before seeing any client information. Your private identifying information will never appear
    in reports.


    I understand that information about services provided to me may be used to conduct research and reporting related to service needs, income support, education and employment, and program effectiveness. The use of this information for research and reporting may last beyond the actual delivery of current services. My name, social security number, or other personal information that identifies me will never appear in research or a report. I also understand that ULSDC may use my phone number, email, or address to contact me about upcoming classes or my progress toward my goals. ULSDC will always provide a method to opt out of calls or emails. My signature shows that I permit you to enter my personal information into CMAX and receive periodic contact from ULSDC via phone or email, with the option to opt-out.


    I grant ULSDC, its representatives, and employees the right to photograph me and my property concerning the workshop's subject. I authorize ULSDC, its assigns, and transferees to copyright, use, and publish the photograph in print and/or electronically.


    I agree that ULSDC may use such photographs of me, with or without my name, for any lawful purpose, including publicity, illustration, advertising, and web content.


    I certify that all information in this intake form is true and correct to the best of my knowledge. I understand that false or misleading information may result in an incorrect analysis of my financial situation. I also understand that after this program, I am not guaranteed to purchase a home, refinance, or receive a mortgage workout.

  • Class Disclosure and Conflict of Interest Policy Statement

    PLEASE READ THOROUGHLY. SIGNING BELOW CONFIRMS YOU HAVE READ AND UNDERSTAND ALL OF THE URBAN LEAGUE OF SAN DIEGO COUNTY (ULSDC) DISCLOSURES AND POLICIES STATEMENTS BELOW. IN ADDITION, BY PARTICIPATING IN ANY OF OUR CLASSES OR PROGRAMS, YOU ARE CONSIDERED OUR CLIENT AND WILL BE REFERRED TO AS OUR CLIENT.

    1. ULSDC provides information and education on numerous loan products, housing programs, and other programs. ULSDC offers the following services: Education (financial literacy, homebuyer education, post-purchase workshops), Counseling (pre-purchase & post-purchase counseling including Foreclosure Counseling, Youth Education & Employment Training, Work Readiness, Elderly Health Access, and Civic Engagement. The information that is received from the ULSDC in no way obligates our client(s) to choose any particular product(s) or housing program that is presented.
    2. ULSDC program administrators and/or their agents may follow up with their client(s) for program monitoring and evaluation. ULSDC collects demographic data and may be asked to share this information with their funders.
    3. The client (s) acknowledge that they have received a copy of the handout “For Your Protection Get a Home Inspection” and the “Ten Important Questions to Ask a Home Inspector.”
    4. ULSDC Housing and Financial Capabilities Program has financial relationships with outside funders. ULSDC client(s) are not obligated to use any of the services provided by their funders. ULSDC Financial Capabilities Program Funders include some of the following: National Urban League , Wells Fargo Bank, Union Bank, JP Morgan Chase, Citibank & Private Donations.
    5. Instructors and/or counselors may answer questions and provide information but not give legal advice. If our client(s) want legal advice, they will be referred to another agency for appropriate assistance. Client(s) are encouraged to promptly consult with service providers, agencies, organizations, or any other legal referral sources to which the client(s) have been referred to follow up on their potential legal issues.
    6. Our client(s) may be referred to other housing services of this organization, another agency, or agencies as appropriate that may be able to assist our client(s) further with their particular concerns that have been identified. ULSDC client(s) are not obligated to use any of the services that are presented.
    7. ULSDC prohibits the following actions to prevent a conflict of interest in providing Housing, Financial Capabilities, and Education services.
    • ULSDC will ensure and monitor that the agency, its staff, or any member of their immediate family must not take any action that may result in or create the appearance of administering the Housing and Financial Capabilities program for personal or private gain, providing preferential treatment to any organization or person, or undertaking any action that might compromise the agency’s ability to ensure compliance with HUD program requirements or to serve the best interests of its client(s).
    • Individual directors, employees, or family members of the ULSDC may not accept a fee or any other consideration for referring a client(s) to mortgage lenders, brokers, builders, or real estate sales agents.

    • Director, employee, officer, contractor, or agent of the ULSDC shall not refer client(s) to mortgage lenders, broker, builders, or real estate agents in which the officer, employee, director, his spouse, child, or general partner has a financial interest, neither may they acquire the client’s property from the trustee in bankruptcy or accept a fee or any other consideration for referring a client(s) to mortgage lenders, brokers, builders, or real estate sales agents or brokers.

    • Director, employee, officer, contractor, agent, his or her spouse, child, general partner, or organization in which he or she serves as an employee (other than with the ULSDC) or with whom he or she is negotiating future employment, may not have a direct interest in the client(s) as a landlord, broker, or creditor, or originate, have a financial interest in, service, or underwrite a mortgage on the client’s property, own or purchase a property that the client(s) seeks to rent or buy or serve as a collection agent for the client’s mortgage lender, landlord, or creditor.

    3. ULSDC provides Housing and Financial Capabilities coaching, after which I will receive a written action plan consisting of recommendations for handling my situation / finances, possibly including referrals to other housing agencies as appropriate.
    4. Client(s) are expected to FULLY PARTICIPATE in the financial coaching process and follow up with their action plan. I agree to allow ULSDC to pull my soft credit report report at the time of intake. In lieu of a new soft credit pull, I agree to provide ULSDC with a copy of credit report dated within 30 days of the intake date.
    5. I understand that ULSDC is required to submit client-level information to the online reporting system and share some of my information with CALHFA administrators or their agents for the purposes of program monitoring, compliance, and evaluation.
    6. I understand that ULSDC funders may need to open files to be reviewed for program monitoring and compliance purposes and conducts follow-ups with clients related to program evaluation. I give permission for ULSDC administrators and/or their agents to follow-up with me between now and June 30,2026, for the purposes of program evaluation.
  • 7.  I understand that I may be referred to other ULSDC programs/services or other agencies or agencies as appropriate that may be able to assist with concerns that have been identified. I also understand that I am not obligated to follow any of the recommendations or use any of the services offered to me. ULSDC leases/rents residential properties to the public. 

    8. I acknowledge that I have received a copy of the ULSDC Privacy Policy. I understand that I may opt out, and that must be recorded in my client file. The client (s) also acknowledge that they have received a copy of Get a Home Inspection, 10 Questions for a Home Inspection, Lead-Based Disclosure Sample, and Radon Fact Sheet.

    9. ULSDC will make reasonable efforts to conduct a verbal follow-up within 60 days of no client contact. If unsuccessful, after two attempts to perform a verbal follow-up with the client, ULSDC will write an email or letter to the client requesting a response. If ULSDC does not hear back from the client, ULSDC will close the case, and no further attempt of contact will be made.

    10. Coaches may answer questions and provide information but not give legal advice. If our client(s) want legal advice, they will be referred to another agency for appropriate assistance. Client(s) are encouraged to promptly consult with service providers, agencies, organizations, or any other legal referral sources to which the client(s) have been referred to follow up on their potential legal issues.

    11. Client (s) understand that if they need service in a language other than English or Spanish, they are welcome to ask for our Referral Translation Service Form.

    12. ULSDC reserves the right to update or change our service delivery model and will do our best to keep you apprised of these changes.

  • PRIVACY POLICY AND PRACTICES

    We at Urban League of San Diego County are committed to assuring the privacy of individuals and/or families who have contacted us for assistance. We realize that the concerns you bring to us are highly personal. We assure you that all information shared, both orally and in writing, will be managed within legal and ethical considerations. Your “nonpublic personal information,” such as your total debt information, income, living expenses, and personal information concerning your financial circumstances, will be provided to creditors, program monitors, and others only with your authorization and signature on the second
    page of this form. We may also use anonymous aggregated case file information to evaluate our services, gather valuable research information, and design future programs.

    Information We Collect

    We collect personal information to support our Housing and Financial Capabilities operations and to aid you in shopping for a home mortgage, sustaining your home mortgage, or attaining housing stability. We collect personal information about you from the following sources:

    • Information we receive from you orally, on applications or other forms, such as your name, address, social security number, date of birth, employer, occupation, assets, debts, and income; and
    • Information about your transactions with us, your creditors, our affiliates, or others, such as your account balance, payment history, parties to your transactions, and credit card usage and
    • Information we receive from a credit reporting agency, such as your credit bureau reports, your credit history
      and your creditworthiness.

    Information We May Disclose

    We may disclose the following kinds of personal information about you:

    • Information that we receive from you on intake or other forms, never date of birth, name or social security;
    • Information about your transactions with us, our affiliates or others,
    • Information we receive from a consumer reporting agency and
    • Information that we receive from personal and employment references.

    To Whom Do We Disclose
    So long as you have not opted out, we may disclose some or all of your personal information that we collect to the
    following types of unaffiliated third parties:

    • Financial service providers, such as companies engaged in providing home mortgages or home equity loans,
    • To your creditors or third parties where we have determined that it would be helpful to you, would aid us in counseling/coaching you to reach your housing goal, or is a requirement of grant awards, which makes our no-cost services possible to your community.
    • We may also disclose any nonpublic personal information about you or former customers to anyone as permitted by law (e.g. if we are compelled by legal process).
    • Others, such as nonprofit organizations, are involved in community development, but only for program review, auditing, research, and oversight.

    Before sharing personal information with unaffiliated third parties, except as described in this policy, we will give you
    an opportunity to direct that such information not be disclosed.

    Confidentiality and Security
    We restrict access to personal information about you to only those employees who need to know that information to provide products and services to you and to help them do their jobs, including underwriting and servicing loans, making loan decisions, aiding you in obtaining loans from others, and financial & housing counseling. We maintain physical and electronic security procedures to safeguard the confidentiality and integrity of personal information and guard against unauthorized access. We use locked files, user authentication, and detection software to protect your information. Our safeguards comply with federal regulations to guard your personal information.

    Directing Us Not to Make Disclosures to Unaffiliated Third Parties

    You may opt out of certain disclosures

    1. You have the opportunity to “opt-out” of disclosures of your nonpublic personal information to third parties (such as
      your creditors), that is, direct us not to make those disclosures.
    2. You may opt out of this requirement, but proof of your decision must be recorded in your client file.
    3. If you choose to “opt-out,” we will not be able to answer questions from your creditors. If, at any time, you wish to
      change your decision about your “opt-out,” you may call us at 619-266-6256 and do so.
