1. Prior to permitting employees to start demolition operations, has a competent person completed an engineering survey of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure?
2. Have adjacent structures where employees may be exposed also been surveyed by a competent person?
3. Has documentation of the surveys above been maintained to show that such surveys have been performed?
4. Prior to initiating demolition activities, has a demolition plan and engineering, asbestos, and lead surveys been completed? (Note: Some jurisdictions may require this to be done by a registered professional engineer.)
5. Have all electric, gas, water, steam, sewer, and other service lines been shut off, verified to be clear and capped or otherwise controlled outside the building line before demotion is started?
6. Has it been determined if any hazardous building materials such as asbestos, lead, mercury, PCBs, hazardous chemicals, gases, explosives, flammable materials, or other hazardous substances were used in any building construction, pipes, or tanks?
7. Have the floors and walls been shored or braced as deemed necessary by a structural engineer?
8. Are all test results from engineers, and others who performed surveys and sampling, on file in their final signed versions?
9. If employees are to work within a structure to be demolished that has been damaged by fire, flood, explosion, or other cause, have the floor and walls been shored or braced as per the engineer’s instructions?
10. Have employee entrances to multistory structures being demolished been adequately protected (e.g. sidewalk sheds, canopies, or both)?
11. Is the designated means of access and egress indicated on the demolition plan?
12. Are continuing inspections by a competent person being conducted during demolition to detect hazards resulting from weakened or deteriorated floors or walls or loosened material?
13. Are chute openings into which debris is dumped protected by a guardrail that is 42 inches (1.07 m) above the floor or other surface on which personnel stand to dump the material?

14. Are signs warning of the hazard of falling materials posted at each side of the debris opening at each floor?
15. Are front window guards used on mobile equipment that is exposed to falling debris or material that could be caught in the tracks of an excavator or other equipment?
16. Has a competent person been assigned to control operation of the chute gate and the backing and loading of trucks?
17. Is the area surrounding the discharge end of the chute closed when operations are not in progress?
18. Are chutes designed and constructed of such strength as to eliminate failure due to impact of materials or debris loaded therein?
19. Do floor openings have curbs or stop-logs to prevent equipment from running over the edge?
20. Are masonry walls, or sections of masonry, prevented from falling in such quantities as to exceed the safe carrying capacities of the floors?
21. Are employees prohibited from working during hazardous weather?
22. Are floor openings within 10 feet (3.05 m) of any wall being demolished planked solid except when the area beneath is barricaded to prevent workers from being exposed to falling objects?

28. Are structural members being dismembered in proper sequence to avoid overstressing and failure?
29. Are all persons prohibited from being in areas affected by demolition when balling or clamming is being done?
30. Is the weight of the demotion ball within 50% of the crane’s rated capacity at the operating configuration?
31. Is the ball attached to the load line with a swivel connection to prevent twisting, and is it protected against accidental disconnection?
32. Are all roof cornices or other ornamental stonework removed before pulling walls over?
