    Deadline to turn in all paperwork is Thursday, October 10, 2025.  No paperwork will be accepted after this date. Your child may not begin sacramental classes without this form being returned.

  • REQUIRED INFORMATION:

  • Requirements:

    • The child must be bqptized in the Catholic Church so PLEASE attach a copy of his/her Baptism Certificate.
    • Must be at least 7 yrs. old by 9/1/25 and in the 2nd grade or higher.
    • Attended Faith Formation classes during the 2024-2025 school year at a Catholic school, parish, or Catholic homeschool program.
    • Parent and child must attend Mass on weekends.
    • Parent must sign and return this Sacrament Preparation Policy form.
    • Parent must attend parent meetings, parent/child retreat, required Adult Faith Formation classes, and any other preparation needed according to Faith Formation Staff.
    • No more than 3 absences for the 2024-2025 school year and the 2025-2026 school year.

  • My child is preparing for the sacraments of Reconciliation and Eucharist. I/we understand that my/our participation in his/her religious training is expected and needed. As a faithful Catholic and responsible parent/guardian...

    1. I will attend Mass each weekend with my child.
    2. I will see to it that the necessary documentation, Baptism Certificate, letter to Fr. Trung Nguyen and this Sacramental Preparation form and fee of $50.00 is turned in and paid no later than October 10, 2025. If your child was baptized at Sacred Heart of Jesus, you will still need to provide us a copy of your child's baptism certificate. Please be sure to put your child's name on all correspondence sent to Faith Formation.
    3. I understand that if my child attended Faith Formation classes the previous year at another parish or a Catholic School, I will provide a letter from that parish/school no later than October 10, 2025
    4. I will encourage attendance and participation in the daily/weekly classes and discuss with my child what he/she has learned from those classes.
    5. I will see to it that my child and at least one parent are present for scheduled retreats/workshops. I will see to it that my child attends all Sacramental Preparation classes (these classes are separate from regular Faith Formation classes). I or the child's other parent will attend all required Adult Faith Formation classes.
    6. I will encourage and pray for my child as he/she prepares for the sacraments of Reconciliation and Eucharist.
    7. I understand that a letter written by the parents to the pastor is required indicating the family's readiness for the child to receive the Sacraments of Reconciliation and Eucharist. This letter should also address how the family practices their faith.

    I have read and understand all of the expectations. If my child and/or I have not met all requirements, then my child may not receive his/her Reconciliation/First Communion.
