Repair Process- CCC Powersports repair process progresses through three stages as follows: 1) Vehicle analysis : in this stage the technician evaluates the unit and creates a report listing all necessary repairs. This report details any damage or outstanding maintenance required, and includes expected / Estimated repair costs to the best of their knowledge. This report will then be used to create the repair estimate with itemized parts & labor costs and be discussed with the customer for approval to move forward with the service/ repair of their unit. 2) Additional damage / Issues: During the repair process, additional problems or issues may be uncovered. in the event that our technician discovers additional issues and determines the need for repairs outside the listed items discussed and approved, work on the unit will cease and the customer will receive an additional phone call to discuss further repairs needed. CCC Powersports will not perform any repairs outside of those listed in the initial estimate without prior verbal or written authorization of the customer to approve the addition repairs needed. 3) Insurance Estimate: In the event that an insurance claim is filed by the customer and the customer has requested that CCC Powersports Perform the needed repairs, CCC will create an insurance estimate for submission to the insurer. This estimate is intended for internal use by the insurer for purposes of validating the insurance claim and reserving funds for payment at the conclusion of the repair services.

Customer is called to discuss the estimate for approval to move forward with the repair

Once the customer approves the estimate parts are ordered

When parts arrive the Tech will start working on the unit

Once the repair is Completed The customer is called and notified that the unit is Finished and Ready for pick-up

By signing below the customer acknowledges that any repairs using parts not made by the vehicle's manufacturer (commonly known as "aftermarket parts") may lead to the following: 1. Manufacturer Warranty: Some vehicle manufacturers require that all repairs be made using only OEM parts. Use of aftermarket parts may void vehicle warranties. In recognition of the above risks, CCC Powersports shall not use any parts for repairs unless those parts are approved for use by the customer. It is advised that the customer take the above risks into account when choosing parts for necessary repairs. While the customer has the sole right to choose which parts are used for repairs to the listed vehicle, CCC Powersports reserves the right to exercise professional judgement and refuse to make repairs using parts which are deemed inadequate or unsafe. Repair Parts Warranty - CCC Powersports will under the terms of this Repair contract, for a period of 30 days after the completion of the authorized repair, at their sole discretion, correct problems (only) related to the repairs performed in relation to this contract. Furthermore, this Claim only covers Manufacturer defects of parts used to complete the authorized repair, and does not cover abuse, wear and tear, user error, neglection or improper use. Any issues found by the customer within that 30 days must be brought to the attention of CCC Powersports immediately. If an issue or problem occurs it is the sole responsibility of the customer to stop the use of the vehicle immediately so no further damaged is caused. CCC Powersport will not be held liable for further damaged caused due to the customer continuing to drive the vehicle.

by signing below I the customer listed above agree to defend, indemnify and hold completely harmless CCC Powersports (Califano Custom Creations) from and against any and all claims, injury, damages, cost, and expenses, including attorney's fees, arising from or related to your use of the Products Repaired.

I hereby grant you (CCC POWERSPORTS) and/or your employees permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of testing and/or inspections. I agree that you are not responsible for loss or damage to vehicles or in case of fire, theft, vehicle fading caused from the suns UV rays, weather related causes or any other cause beyond your control. I authorize CCC Powersports to take the steps necessary to perform the repairs listed in this contract. I acknowledge that CCC Powersports has no formal relationship or obligation to any other person and is not obligated to communicate with or assist any other person, including discussing any cost estimates, repair information or unit problems with anyone other than the name listed on this contract. I agree to pay in full for all services performed. I understand that certain estimates and fees must be prepaid if requested by CCC. I am acknowledging that I have been informed of and agree to CCC Powersports shop rates. (The Shop rate for UTVs, ATVs, Go-karts, Dirt Bikes, Scooters, Mini Bikes & Motorcycles @ $95 per hour) I agree to pay for all parts, labor, shop fees, freight, taxes, and any other charges incurred during the performance of any repairs that I have authorized either in writing or verbally by phone or in person. In the case that I do not approve the suggested repair or service estimate I will still owe an Estimate fee of $95 (or more if extra time was approved) for the services rendered and time spent determining the provided Estimate. I understand any cost quoted previously is an estimate only. CCC Powersports will provide an invoice upon completion of repairs detailing all fees, and listing a final outstanding balance for payment. Payment in full is expected on completion of repairs. Vehicles will be released on full payment. An express mechanic's lien is hereby acknowledged on above vehicle to secure the amount of repairs. All vehicles left over 14 days after repairs are completed will incur a $50.00 per day storage fee. We accept cash and most major credit cards. In the event that repair services rendered are not paid for within 30 days of completion, CCC Powersports reserves the right to sell my vehicle to recover the costs incurred while performing the approved vehicle repair services. This contract shall be considered binding upon both parties and their assignees or successors , including subsequent title holders for the vehicle being repaired. My signature below indicates approval of services and rates listed in this repair contract.

The technician assisting you with the check-in process will help you with this portion of the check-in. He will do a full walk around of your unit and will take note of any visual damage that is visible at the time of check-in such as anything scratched, dented or broken on your unit. He will also take a full 360 degree set of pictures of your vehicle to document the condition it is in at time of drop-off at our location. Once the technician is finished, you will sign the bottom of this form to verify that all notations and information noted is true and correct.
