2025/2026 Science & Social Studies Registration

Thank you for your interest in the Science and Social Studies Program!

Please complete ALL THREE steps to enroll.

1. Fill out one registration form entirely per student.

2. Upon email confirmation of enrollment, pay a one-time $25 campus enrollment fee per student.

*Fee is not covered by charter funds. It is a one-time per school year fee.

*If you have paid the yearly enrollment fee as part of the 2025/2026 Vendor Campus, you do not need to pay again.

3. Submit your charter order in July when funds drop or private pay by August 1, 2025.

*Approved charter certificate or private payment BEFORE class begins secures your student's spot in the class. Any enrolled student's without payment will forfeit their spot to someone on the waitlist. Thank you for understanding!

Waiver of Liability, Media Release, & Technology Policy
Waiver of Liabiity

I, {primaryParentguardian}, parent or legal guardian of {studentName} give my full permission for my child to participate in classes and activities at Enlightened Minds. In allowing my child to participate in classes and activities at Enlightened Minds, I fully and completely release, discharge, and hold harmless Enlightened Minds, and its directors, trustees, officers, employees, agents, insurers, instructors, and volunteers (collectively "its agents and employees) from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury, death, or property damage, arising out of my child's participation in classes and activities at Enlightened Minds.

I acknowledge and recognize there are inherent risks involved in such activities and I assume the risk of any injury sustained while at Enlightened Minds. I agree to indemnify, defend and hold harmless Enlightened Minds and its agents and employees from any and all claims arising out of my child's participation in any class or activity at Enlightened Minds, even if such claim arises as a result of a negligent act or omission of Enlightened Minds or its agents and employees.

Media Authorization and Release

Enlightened Minds requests permission to photograph and/or record your child while participating in classes and/or activities and for the resulting images and/or recordings to be used for internal newsletters, social media marketing, and/or training purposes.

Technology Policy

I, {primaryParentguardian}, parent or legal guardian of {studentName} understand that cellphones, tablets, smartwatches, laptops, and similar devices are not allowed to be used for entertainment purposes while my child is on campus. Cellphones and smartwatches should only be used to communicate with parent or legal guardian and should be placed on silent during my child's time at the learning center to foster a focused and distraction-free learning envrionment and to ensure student safety and well-being for all students on campus. Tablets, laptops, and similar devices should only be brought and used for educational purposes when my child will be doing academic work.

I understand that if my child brings any of these devices to campus, I am responsible to share the expectation of this policy with them. I understand that if my child has a hard time following this policy, Enlightened Minds may ask that my child not bring the device or another alternative be followed.

Monthly Enrollment: I understand Enlightened Minds offers a monthly class, and if I decide that I will no longer enroll my child for the following month, I will extend the courtesy of giving Enlightened Minds a 30-day notice, so that our spot in the class can be made available to another student.

Missed Work: I understand that Enlightened Minds will only provide worksheets when my student is absent. I further understand that it is my responsibility to request those missed worksheets the week my student returns to class.

Communication: The primary means of communication will be through email. If I have any questions about the program or activities I will reach out via email to info@enlightenedminds.net or by phone at (559) 569-1375.

Drop off/Pick up: I will arrive on time and pick up on time. I will notify Enlightened Minds if my child will be absent, late, or if I will need to request an early pick-up.

Water, Snacks, and Lunch: I will send my child with a refillable water bottle, snacks, and if needed a lunch. I will make sure to communicate any allergies or sensitivities which may affect my student or the class.

Holidays/Emergency/Class Cancellations: I am aware that some of the class days may fall on a holiday and that some class days may close due to holidays. In the event of closure due to emergency or other unforeseen circumstances, I am aware that there are no make-up days.

Behavior & Conduct: I will do my best, within my abilities as the parent/guardian, to encourage my student to be prepared for class and ready to participate with a positive attitude. I will instruct my child to show respect for Enlightened Minds instructors, helpers, and other students. I will share with them the expectation that they follow the rules, which includes instructing them to remain with their group and not wander off into other areas. I understand that if there are ongoing behavioral issues or other concerns regarding my child's participation and/or behavior in the class, Enlightened Minds may come to the conclusion that the program is not a good fit for my child, and that they may be dismissed from further instruction.
Financial Responsibilities
Monthly Class Fee: I understand that I am responsible for my child's monthly class fee, which must be received by the 1st of the month and that they may not be allowed to attend class if the payment has not been received and/or confirmed.

Charter School Students: I understand that it is my responsibility to submit the vendor service order for the monthly fee in a timely manner. This usually requires that I submit the order two (2) weeks prior to the start of class, in order for Enlightened Minds to receive my approved certificate before class begins. If Enlightened Minds does not receive my approved certificate by the first of the month, I understand that my student may not be allowed to attend class and their spot will be made available to a student on the waitlist.

I also understand that it is my responsibility to be informed of my charter school's specific guidelines regarding vendor service orders. I understand that if I request services from my charter for use at Enlightened Minds, and I do not utilize the services, that my charter school may not issue me a refund for unused services, and that it is not the financial responsibility of Enlightened Minds to reimburse me for unused services.

Property Damage: I understand that if my child is found to be responsible for any damage to the property where classes are held, I may be financially responsible for the damages.

Refunds/Credits: I understand that there are no refunds or credits for missed days due to illness, vacation, or any other circumstance. If there are closures due to unforeseen events, I understand that we may not have the option for a make-up day.
