  • Waiver of Liability

    We as parent(s) or guardian(s) understand that the school will, on occasion, arrange for opportunities of learning and exploration that will not take place on school premises throughout the 2025-2026 academic school year. As a result, we hereby do grant permission for our students to attend. 

    Notice to parent(s)/guardian(s):

    In the event of a planned field trip, a separate permission form will be provided and required for student participation. The field trip form will include specific details surrounding the experience in addition to requesting an emergency contact in the event of an emergency on the date of the scheduled field trip.

    To participate in such activities, we further understand the risks involved in such activities and agree not to hold the school or diocese responsible for accidents or injury in the event that they may occur.

  • Photo Release Form

    Throughout the 2025-2026 academic school year, your child(ren) may be involved in learning opportunities, activities, events, etc. where photographs are taken and shared with the public. Photos and/or videos of student(s) may be shared in a variety of ways including, but not limited to: Classroom or school websites, social media (ie. Facebook), newspaper outlets or diocesan publications (ie. The Courier).

  • RaiseRight (Scrip) Permission Form & Waiver

    I, hereby give permission to Sacred Heart School to deliver RaiseRight Scrip purchases which I have ordered to my child to transport from school to my home. Furthermore, I certify that I have discussed the responsibilities associated with the transportation of my RaiseRight Scrip with my child. I further understand that I have the option to personally pick up my RaiseRight Scrip order from Sacred Heart School versus rely on my child to accept responsibility to transport it. 

    I agree that once Sacred Heart School has delivered the RaiseRight Scrip purchase to my child, Sacred Heart School is no longer responsible for any RaiseRight Scrip purchase that is lost, stolen or misplaced. I hereby waive any right of recovery that I may have against Sacred Heart School for any RaiseRight Scrip which is lost, stolen or misplaced after it is given to my child. This agreement is effective for the 2025-2026 academic school year.

  • District Pupil Health Services Request Form

    For the 2025-2026 academic school year, the State of Minnesota has authorized local public school districts to allow pupils attending non-public schools (ie. Private, religious, home school, etc.) established and operating within the school district’s boundaries, access to existing district Pupil Health Services through  public health programming. These services must be requested by, or on behalf of, the pupil no later than Tuesday, September 2, 2025.

  • Textbooks, Standardized Tests & Individualized Instructional Materials Request Form

    The State of Minnesota has authorized local public-school districts to loan textbooks, standardized tests, and individual instructional materials to pupils attending a non-public school, including home schools, established and operating within the school district’s boundaries. These materials must be secular in nature; designed primarily for individual use by the pupil in a particular class or program in the school the pupil regularly attends. Requests must be made on behalf of the designated pupil no later than September 2, 2025.

  • Internet Usage Agreement Form - Parent

    Please read before signing. Sacred Heart School has adopted an Acceptable Use Policy to cover the ethical code which has been approved by both the faculty and the Board of Education. 

    • I understand that it is impossible for Sacred Heart School to restrict all controversial materials, and I will not hold them responsible for materials acquired on the Internet. 
    • Further, I accept full responsibility for supervision if and when my student’s use is not in a school setting.
    • I hereby give permission to provide access for my student on the Internet and certify that the information contained on this form is correct.
    • I accept that Sacred Heart School has adopted a policy that strictly prohibits any financial activity via the Internet. If my child breaks this agreement I accept responsibility for any financial commitments he/she made.
