  • MEDICAL CONCERNS

    At our camp, the safety and well-being of every child—and every volunteer—is our top priority. Because we work with children ages 2–7 who have disabilities and/or speech delays, we strive to create a safe, supportive, and inclusive environment for everyone involved.Please let us know if you have any medical conditions that could affect your ability to participate in camp activities. This includes (but is not limited to): Physical fitness,Recent or chronic injuries,Allergies (food, environmental, medication, etc.), Physical or emotional limitations, Any other medical or mental health conditions. Your information will be kept confidential and is only used to ensure we can provide the safest and most supportive experience possible.

    If your records have been expunged pursuant to applicable law, you are not required to answer yes to the following questions. If you are unsure whether to answer yes, we strongly suggest that you answer yes and fully disclose all incidents to avoid any future risk of embarrassment upon disclosure.
  • REPRESENTATION AND RELEASE

    I understand that The Free Play Foundation Inc. (the “Foundation”) is committed to creating a safe, inclusive, and nurturing environment for children with disabilities and/or speech delays, ages 2–7. As a volunteer, I acknowledge that I will be expected to follow all guidelines, policies, and procedures established by the Foundation, including strict policies prohibiting any form of abuse, neglect, or inappropriate conduct. I understand that any suspected abuse or misconduct—whether child-to-child, volunteer-to-child, or staff-to-staff—must be immediately reported to a Foundation supervisor or staff member. All reporting is kept confidential, in accordance with the law. I understand that all volunteers are required to complete and pass a background check prior to acceptance. I further acknowledge that failure to complete this process, refusal to cooperate, or an unsatisfactory background check may result in disqualification or dismissal as a volunteer. I certify that all the information I have provided in this application (and any related documents) is true and complete to the best of my knowledge. I understand that misrepresentation, falsification, or omission of information may be grounds for denial or immediate removal from the volunteer program, regardless of when discovered. In consideration of my application, I authorize The Free Play Foundation Inc. to contact references, previous employers, schools, volunteer organizations, or any other sources to obtain information regarding my qualifications and character for working with children. I release all individuals and organizations contacted from any and all liability for providing such information. I also agree to hold harmless The Free Play Foundation Inc., its staff, board, volunteers, and affiliated partners from any claims or damages that may arise from the collection or use of this information. Furthermore, I release The Free Play Foundation Inc., its directors, officers, staff, agents, and volunteers (“Released Parties”) from any claims or liability for injury, death, loss, or damage that may result from my participation as a volunteer—except in cases of gross negligence or willful misconduct by the Foundation. I agree not to take legal action against any Released Party and to hold them harmless from costs and damages, including attorney fees, that arise from my violation of these terms. I understand that submitting an application does not guarantee me a volunteer position. If accepted, my role as a volunteer is at-will and may be terminated at any time by either me or The Free Play Foundation Inc., with or without cause or notice. I acknowledge that volunteering with The Free Play Foundation Inc. is not a contractual relationship and does not imply employment. I agree to comply with all Foundation policies, expectations, and leadership direction while volunteering. I understand that my acceptance is based on alignment with the Foundation’s mission and values, and that all decisions related to volunteer roles are at the discretion of the Foundation’s leadership. I have carefully read and understand this Representation and Release. I sign it voluntarily and acknowledge that it is legally binding.
