Nominate up to three K-12 or college age youth sports programs in Ashland, Talent, or Phoenix by June 1, 2026.
To be eligible groups must:
- Be tax-exempt either as a nonprofit organization or school group
- Serve children and youth in Ashland, Talent, or Phoenix
- Be based in Ashland, Talent, or Phoenix
- Be pre-approved by their school district, if applicable*
*Groups from the Ashland and Phoenix-Talent School districts must have approval from the district prior to submitting an application.
