Elite Academy Program – Spring 2025
This program is designed for players born between 2015 and 2010 who are ready to take their skills to the next level. Our Elite Academy focuses on:

Advanced technical training
Tactical awareness & positional play
High-level, competitive environment
📍 Location: Newton Athletic Park - field #4 TURF
📅 Dates: April 11 – June 20
⏰ Time: 6:30 PM – 8:00 PM
💲 Cost: $250

Led by experienced coaches, each session is structured to challenge and develop players both technically and mentally.

RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND INDEMNITY AGREEMENT (YOUTH PLAYER)

I, the undersigned, as the legal parent or guardian of the youth player named below, hereby agree to the terms and conditions outlined in this Release of Liability, Waiver of Claims, and Indemnity Agreement (“Agreement”) with Gabe’s Academy and its representatives.

1. Acknowledgment of Risks: I acknowledge that participation in soccer activities involves inherent risks, including, but not limited to, the risk of injury or death. I understand that Gabe’s Academy will take reasonable measures to ensure safety, but it is impossible to eliminate all risks.
2. Release of Liability: In consideration for allowing said player to participate in soccer activities with Gabe’s Academy, I hereby release, discharge, and hold harmless Gabe’s Academy, its officers, directors, coaches, staff, volunteers, and any other representatives from any and all claims, liabilities, demands, actions, or causes of action, whether in law or in equity, arising out of or related to any injury, illness, or harm that may be sustained by said player during soccer activities.
3. Waiver of Claims: I waive any and all claims or causes of action against Gabe’s Academy arising out of or related to said player’s participation in soccer activities, including but not limited to claims for negligence, breach of contract, or breach of any statutory or other duty of care.
4. Indemnity: I agree to indemnify and hold harmless Gabe’s Academy from any and all liabilities, claims, actions, damages, costs, or expenses, including attorney fees, arising out of or related to said player’s participation in soccer activities.
5. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of British Columbia.
6. Consent: I hereby consent to Gabe’s Academy providing necessary medical treatment to said player in the event of an emergency, and I agree to be responsible for any medical expenses incurred.

PARENT/LEGAL GUARDIAN CONSENT RELEASE: PHOTO, VIDEO, FILM & SOUND RECORDING

I, the undersigned, as the legal parent or guardian of the youth player named below, born on [Date of Birth], hereby grant permission to GABE’S ACADEMY to photograph, videotape, film, and record sound (“Media”) of said youth during soccer activities organized by GABE’S ACADEMY .

1. Consent for Media Use: I understand that the Media may be used by GABE’S ACADEMY for promotional, educational, or informational purposes, including but not limited to, websites, social media, brochures, advertisements, and other materials.
2. Release of Rights: I hereby grant GABE’S ACADEMY the irrevocable and unrestricted right to use, reproduce, publish, and distribute the Media, in whole or in part, without compensation to me or said youth. I understand that GABE’S ACADEMY retains the right to use the Media indefinitely.
3. Protection of Privacy: I acknowledge that GABE’S ACADEMY will make reasonable efforts to protect the privacy of said youth and the undersigned. However, I understand that GABE’S ACADEMY cannot guarantee complete privacy, and I release GABE’S ACADEMY from any liability for the use or disclosure of the Media.
4. Withdrawal of Consent: I understand that I have the right to withdraw this consent at any time by providing written notice to GABE’S ACADEMY . Any withdrawal of consent will apply to future use of the Media but will not affect any prior use.
5. Governing Law: This Consent Release Form shall be governed by and construed in accordance with the laws of British Columbia.
