Model/Performer understands that all the information given in this Record-Keeping Compliance Form is being provided to comply with federal law and any false statement will subject Model/Performer to both civil action by The Photographer/Producer as well as possible criminal prosecution under federal and state laws.

SWORN STATEMENT: UNDER 28 U.S.C.1746 AND THE PENALTIES OF PERJURY UNDER THE LAWS OF THE UNITED STATES, I SWEAR THAT THE FOREGOING IS TRUE AND CORRECT AND THAT EACH OF THE IDENTIFICATION DOCUMENTS WHICH I HAVE PROVIDED AND OF WHICH I HAVE SIGNED THE ATTACHED COPY WAS LAWFULLY OBTAINED BY ME AND HAS NOT BEEN FORGED OR ALTERED.

MODEL RELEASE AGREEMENT - TERMS AND CONDITIONS

I, the undersigned model ("The Model"), voluntarily consent to the creation and use of photographs, video recordings, and digital or electronic reproductions of myself—whether clothed, nude, or sexually explicit in nature ("Content").

I grant Rakehellguzart Photography and Helder Guzman ("The Photographer"), along with his heirs, legal representatives, assigns, and those acting under his authority, the perpetual and irrevocable right to use, reproduce, alter, publish, commercialize and distribute the Content, in whole or in part, in any format and for any lawful purpose. This includes, but is not limited to, use for editorial, advertising, trade, exhibition, video production, resale, and digital or print publication across all media platforms, including websites and social media.

The Content may be modified, combined with other materials, and used under my name or a fictitious one. I waive any right to inspect or approve the final use or context in which the Content appears.

Compensation and Legal Waiver

I waive any right to compensation, royalties, or financial gain from the use or distribution of the Content now or in the future. I release and discharge The Photographer and his agents from all claims, demands, or liability related to the creation or use of the Content, including any claims for defamation, invasion of privacy, or infringement of moral rights. I affirm that I am of legal age and that I have agreed to participate—alone or with others—in the creation of the Content, that may include nude and/or sexually explicit material, of my own free will and full consent.

Acknowledgment and Consent

I confirm that I have the legal capacity to enter into this agreement. I have read and fully understand its contents and acknowledge that it is binding upon me and my heirs, legal representatives, and assigns.

I understand that some Content may include adult themes, including real or simulated acts and I give my full consent to participate in such material and confirm that I will not be offended by its use or by the participation of others.

I understand that I may use part of the Content provided to me by The Photographer for personal promotional purposes or in my personal creator platforms giving the corresponding credit to the Photographer, mentioning him, his business and using his social media tags.

By signing below, I affirm that all information provided is true and that I fully agree to the terms and conditions set forth in this agreement.
