• Blue Alchemy Freediving - Liability Release & Assumption of Risk

    Welcome, Please read this carefully and sign at the bottom. This agreement helps ensure clarity, safety, and shared understanding for all who train with Blue Alchemy Freediving. If you have questions, you're encouraged to reach out before signing.

  • Acknowledgment of Risk
    I understand that freediving and breath-hold activities carry inherent risks, including but not limited to:

    • Hypoxia, blackout, drowning, and brain injury
    • Equipment malfunction or misuse
    • Marine life injuries or environmental hazards
    • Injuries entering or exiting the water
    • Remote locations with limited access to medical care
    • Failure of others to render aid

    I understand these risks exist even with proper training and safety protocols, and I voluntarily choose to participate.

    Release of Liability
    In consideration of participating in any Blue Alchemy Freediving activity, I agree to:

    • Release and discharge Blue Alchemy Freediving, its instructors, staff, affiliates (including PFI and Molchanovs), from any liability for injury, death, property damage, or other loss, even if due to negligence.
    • Indemnify and hold harmless all released parties from any claim made by me or a third party.

    Health Declaration
    I affirm that:

    • I am in sound physical and mental condition.
    • I am not under the influence of alcohol, drugs, or any substance contraindicated for freediving.
    • I am solely responsible for ensuring that any equipment I use is safe and functional.

    Legal Acknowledgment
    I understand that:

    • Any legal claim must be filed within one (1) year of the incident.
    • I waive my right to a jury trial, and agree that any dispute shall follow the laws and jurisdiction of the state in which Blue Alchemy Freediving operates.

  • Payments, Cancellations, and Rescheduling. Course payments, cancellations, transfers, required preparation, and missed-course terms are governed by the Blue Alchemy Freediving Payments, Cancellations, and Rescheduling Policy: https://www.bluealchemyfreediving.com/cancellation

  • I,   *   *   , hereby authorize Blue Alchemy Freediving and its affiliated instructors, contractors, and partners to use my likeness in photographs, video, and other media in any and all of its publications, including but not limited to printed and digital formats, websites, social media, and promotional materials. I understand and agree that any media containing my likeness will be the property of Blue Alchemy Freediving.


    I acknowledge that my participation in Blue Alchemy Freediving activities is voluntary and that I will receive no financial compensation for the use of my likeness.


    I hereby irrevocably authorize Blue Alchemy Freediving to edit, alter, copy, exhibit, publish, or distribute any such likeness of me for purposes of publicizing its programs, business, or for any other lawful purpose. I waive the right to inspect or approve the finished product, including written or electronic copies, in which my likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of my likeness.


    I hereby hold harmless and release and forever discharge Blue Alchemy Freediving, its instructors, officers, directors, affiliates, and agents from all claims, demands, and causes of action which I, my heirs, representatives, executors, or administrators, or any other person acting on my behalf, or on behalf of my estate, have or may have related to this authorization and use of my likeness.


    I affirm that I am at least 18 years of age and competent to contract in my own name. I have read this release before signing and fully understand its contents, meaning, and impact
