This is the sign-up form for 036 teams who are joining the Hemelvaartternoi.
Each team should have a captain assigned, have all team members join the 036 Whatsapp group, have registered directly with the tournament (remember to have “036” in your team name) and have completed the below t-shirt form before 5 May. Once the quantity of t-shirts is confirmed, we will confirm the cost per shirt which needs to be collected by the team captain.
