By completing and signing this form, you acknowledge and accept all terms in the Christian K9 Academy (midwest working dogs) Liability Waiver. Property Address: 5050 Goodson Rd, West Jefferson, OH 43162.
1. Voluntary Participation: I voluntarily choose to enter the premises of Christian K9 Academy, located at 5050 Goodson Rd, West Jefferson, OH 43162, and to participate in activities which may include but are not limited to dog training, dog handling, demonstrations, and/or other events or use of the property.
2. Acknowledgement of Risks: I understand that working with dogs—especially working or protection dogs—carries inherent risks, including the risk of injury from dog bites, scratches, sudden movements, or unpredictable behavior. I also understand that there may be other risks involved in being on the premises, including but not limited to slips, trips, falls, and other physical injuries.
3. Assumption of Risk: I acknowledge these risks and accept full responsibility for any injury or harm that may occur to myself, my property, or anyone accompanying me while on Christian K9 Academy property or participating in any related activities.
4. Release of Liability: I hereby release and hold harmless Christian K9 Academy, its owners, employees, contractors, agents, and affiliates from any and all liability, claims, demands, causes of action, or expenses (including attorney's fees) arising from or related to any injury, loss, or damage to myself, my companions, or my property during my presence on the property or participation in any activities.
5. Indemnification: I agree to indemnify and defend Christian K9 Academy from any claims brought by any third parties related to my actions or conduct while on the premises.
6. Medical Emergencies: In the event of an injury, I authorize Christian K9 Academy to seek emergency medical treatment on my behalf, if necessary, and agree to be responsible for any associated costs.
7. Photography/Videography: I grant permission for Christian K9 Academy to use any photographs or video taken of me or my dog(s) during training or events for promotional or educational purposes unless I specifically request in writing that they not be used.
8. Minor Participants: If I am the parent or legal guardian of a minor (under the age of 18) who is present or participating in any activity on the property, I agree to the terms of this waiver on their behalf and accept full responsibility for their safety and conduct. I understand that the risks stated above also apply to minors, and I agree to release and hold harmless Christian K9 Academy from any and all liability related to a minor under my care.
9. Binding Agreement: I agree that this Waiver shall be binding upon me, my heirs, legal representatives, and assigns.
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY AND FULLY UNDERSTAND ITS TERMS. I SIGN IT FREELY AND VOLUNTARILY.
I have read and agree to the Christian K9 Academy Liability Waiver and Release of Claims. I understand the risks involved in handling and being around dogs and voluntarily accept full responsibility for myself and any minors accompanying me.
