To Schedule a Patch Test:
Please contact Edessa Studio (the "business") by text at (909) 524-9121 soon after booking your brow appointment. Patch test must be conducted at least 48 hours prior to scheduled appointment, even with given availability. Failure to do so is consent to waiving patch test.
Patch Test Explanation:
I understand that a patch test is recommended prior to undergoing a brow lamination and/or tinting treatment. A patch test involves applying a small amount of the products that will be used during the treatment to a small area of skin to check for any adverse reactions. This is done to ensure my safety and to minimize the risk of allergic reactions.
Potential Reactions:
- Allergic reactions such as itching, redness, swelling, or rash.
- Irritation or discomfort in the treated area
- Anaphylaxis
Consent:
I hereby expressly waive and release any and all claims, now known or hereafter against Edessa Studio (the "business") and its successors, and assigns (collectively, "Released") on account of personal or psychological injury, illness, pain, suffering, temporary or permanent disabilities, allergic reactions, undesirable results, death, property damage, or financial loss arising out of or attributable to my being on the premises and receiving certain spa and/or beauty services, whether arising out of the ordinary negligence of the business or any Releasees or otherwise.
