We require the form be filled by returning members and new applicants in order to keep our membership data up-to-date. This is an application and does not guarantee acceptance into the choir. The conductor of the choir will determine acceptance.

The organization will not tolerate any form of abuse or bullying, including but not limited to physical, verbal, emotional abuse or harrassment. Any individual who violates this policy will be subject to diciplinary action up to an including termination of membership.

In accordance with Alberta FOIP regulations, all personal information of the applicant/parent/guardian shall be kept confidential and disposed of appropriately when no longer needed. Only persons with the need to now shall have access to the information herein.
