    A CREDIT CARD MUST BE PLACED ON FILE AT THE TIME OF BOOKING.

    Appointments are confirmed through text, email or phone call depending on your preference. However, if we are unable to reach you, please understand that it is your responsibility to remember your appointment date(s) and time(s).

    The below charges will be charged to the Credit Card we have on file for you.

    • Your appointment will automatically be canceled and considered a No Show if you are more than 15 minutes late to your appointment.
    • If you do not show for your appointment you will be charged 100% of the reserved service amount, and will not be able to reschedule without a deposit.
    • Appointments that are not canceled within 48 hours will be charged 100% of the reserved service amount.

  • It is my choice to receive services. I understand that any information given is strictly confidential and will be used for no other purpose than to assist the technician in providing a suitable treatment which would take into consideration to my specific requirements. I also understand that failure on my part to disclose information could result in injury and/or illness and I hereby release American Salon and Spa, the technician, Aveda Corporation, and its parent company from any claims resulting from such. I release any responsibility and/or liability concerning the application, processing and consquences of the chemical procedure of my hair. Any information provided to me by the technician is for general educational purposes only and is not intended for any medical purpose.
