Welcome to the CA Child Care R&R Network/Thriving Families CA Foundation, Joint Annual Conference workshop/presenter online application form for the Virtual Conference. The theme of this year's conference is The Power of Partnerships: Uniting to Create Impact. Thank you in advance for volunteering your time and resources to present at our conference. Please read the following instructions before you begin completing this form:

1. JotForm will save your progress whenever you hit the "Save" button at the bottom of each page AND enter your email into the pop-up/click the "send link" button. The first time you click "Save" a pop-up will ask you to sign-in/create a JotForm account. You can bypass this step by selecting "Skip Create an Account" in small print at the bottom of the pop-up. You will then be directed to the next pop-up window to enter your email to receive your link. Check your email to make sure you received the link before closing the form. You can resume where you left off by using the link emailed to you by JotForm. PLEASE NOTE: You must "Save" and enter your email addresses for a new link each time you would like to save and resume. We encourage you to "Save" regularly as you are completing the form. Your links will expire within 90 days or once you "Submit" the form to us, whichever comes first (see step 2 below).

2. Select “Submit Form” once you have completed all sections of the proposal and have reviewed it for accuracy. If you skip this step, your proposal will be inaccessible to the Conference Program Committee and therefore not considered for the conference.

3. Once your form is submitted, a copy of your submission(s) will be emailed to the "Main Contact Email" entered on the next page. This is the best way for you to print your answers.
