THIS WAIVER and RELEASE is made by and between "Participant" and the Coeur d'Alene Cycling Club, Inc. ("CDACC")

ACKNOWLEDGEMENT OF RISKS

The Participant understands and acknowledges that:* This field is required.

Bicycle riding involves inherent risks and dangers of serious bodily injury, including permanent disability, paralysis, and death.

These risks may be cause by the Participant's own actions, inactions, the actions of others participating in the activity, road conditions, weather conditions, or the negligence of the Released Parties named below.

There may be other risks and social and economic losses either not known or not readily foreseeable at this time.

ASSUMPTION OF RISK AND RELEASE

In consideration of being permitted to participate in group bicycle rides organized by CDACC, the Participant:* This field is required.

Voluntarily agrees to assume all risks associated with participation in group bicycle rides, including but not limited to falls, contact with other participants, effects of weather, traffic, and road conditions.

Hereby releases, waives, and discharges the CDACC, its officers, directors, employees, volunteers, agents, and other participants (collectively "Released Parties") caused in a whole or in part by the negligence of the Released Parties.

Agrees to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost that may occur due to Participant's presence or participation in group bicycle rides.

SAFETY AGREEMENT

The Participant agrees to:* This field is required.

Wear a properly fitted and secured bicycle HELMET at all times while riding.

Follow all traffic LAWS and group ride protocols.

MAINTAIN their bicycle in safe operating condition.

Notify ride leaders of any medical conditions that may affect their participation.

Not participate if not prepared for long rides, climbing hills, weather, hydration and nutrition.

MEDICAL TREATMENT AUTHORIZATION

The Participant authorizes CDACC to obtain medical treatment in the event of injury, accident, or illness during group rides.

PHOTO AND MEDIA RELEASE

The Participant grants permission to CDACC to use their likeness in photographs or videos in any and all publications and marketing materials.

ACKNOWLEDGEMENT OF UNDERSTANDING

The Participant has read this waiver, fully understands its terms, and acknowledges that they are signing it freely and voluntarily, and intend by their signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.
