Flourish Speaker Agreement & Media Release ConsentBy submitting this form, I acknowledge and agree to the following terms as a paid featured speaker in the Flourish: Faith, Finances, Fitness & Fulfillment Facebook Live Panel hosted by Coach Evonn:1. Speaker ParticipationI agree to participate as a paid featured speaker/panelist and provide information relevant to the topic assigned to me. I understand that this is a collaborative empowerment event, and I commit to showing up with excellence and professionalism.2. Use of Name, Image, and Likeness. I grant permission for Coach Evonn Firms and her team to use my name, professional bio, headshot, and likeness in promotional materials, including but not limited to social media posts, event graphics, email marketing, and replays. 3. Recording & Replay Rights. I understand that the event may be recorded and repurposed (in whole or in part) for future promotions, digital content, or replay distribution. I grant Coach Evonn Firms full permission to record, edit, and share my contribution for these purposes without expectation of further payment or compensation. 4. Payment & Non-ExclusivityI understand that this is a paid feature opportunity and that I have submitted payment to be included as a speaker. I acknowledge that this agreement does not imply exclusivity and does not limit me from future speaking or business engagements outside this event. 5.
