Ascension Elite Football Camp Registration Form

LIABILITY WAIVER

 

This Waiver and Release of Liability ("Agreement") is executed in consideration of being permitted to participate in the Ascension Elite Football Camp ("Camp"). By signing this document, the undersigned acknowledges and agrees to the following:

 

1. Assumption of Risk

 

I understand and acknowledge that football is a physically demanding sport involving inherent risks, including but not limited to collisions, falls, injuries, fractures, concussions, heat-related illnesses, and other unforeseen injuries. I knowingly assume all risks associated with participation in the Camp.

 

2. Release and Waiver of Liability

 

I, the undersigned, on behalf of myself (or my minor child), hereby release, waive, discharge, and hold harmless Ascension Elite Football Camp, its organizers, coaches, trainers, sponsors, affiliates, staff, volunteers, and representatives (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, injury, illness, or death arising out of or related to participation in the Camp, whether caused by negligence or otherwise.

 

3. Medical Treatment Authorization

 

I authorize the Camp’s medical staff, including on-site EMTs, to administer first aid and/or seek medical treatment for me (or my minor child) in the event of an injury or medical emergency. I understand that I am solely responsible for all medical expenses incurred.

 

4. Insurance and Responsibility for Medical Costs

 

I acknowledge that the Camp carries insurance that provides coverage for injuries up to a certain amount. Any medical expenses that exceed this coverage amount will be the sole responsibility of the participant and/or their parent/guardian. I affirm that I (or my minor child) am covered by a personal health insurance policy to cover any additional costs that may arise due to participation in the Camp.

 

5. Code of Conduct

 

I agree to abide by all Camp rules, regulations, and instructions provided by the Camp staff. I understand that failure to comply may result in removal from the Camp without refund.

 

6. Media Release

 

I grant permission for Ascension Elite Football Camp to use my (or my child’s) image, likeness, and voice in promotional materials, including but not limited to photos, videos, and social media posts, without compensation.

 

7. Parental/Guardian Acknowledgment (For Participants Under 18)

 

I, the parent/legal guardian of the above-named minor, have read and fully understand this waiver and voluntarily agree to its terms on behalf of my minor child.

 

8. Acknowledgment of Understanding

 

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITH FULL KNOWLEDGE OF ITS LEGAL SIGNIFICANCE.
