- Match your community with aligned prospective members during my Community Connections consultation calls
- Ensure that I’m representing your project accurately in general
(I've chosen to use Jotform because it allows both you and me to edit this information whenever we need, and all changes will automatically update the profiles as your project develops.)
Important Notes:
- Time required: 20-40 minutes (approximately 40 questions across 10 pages)
- Progress saving: The form is set up so you can save your responses as you go
- Submission: I won't receive your responses until you click "Submit" on the final page (so please click Submit on the final page ;-)
- Flexibility: Most questions are optional, but brief answers are better than no response. It'll prompt me to elaborate if needed.
