Informed Consent and Acknowledgement
I give permission for my child to participate in all basketball training activities organized by Grind Basketball during the selected camp, clinic, or training session.
I understand that participation in sports activities involves inherent risks, including but not limited to physical injury and risks associated with travel to and from training sessions, games, or events. I voluntarily assume all such risks and accept full responsibility for any injury, loss, or damage that may occur.
I agree to release and hold harmless Grind Basketball, its owners, staff, coaches, volunteers, contractors, and any associated facilities or partners from any and all claims, liabilities, or demands arising from my child’s participation, except where prohibited by law.
I understand that basketball, like any sport, carries risks such as sprains, strains, fractures, concussions, serious injury, paralysis, or death. I acknowledge these risks and give my informed consent for my child’s participation.
Media Release and Acknowledgement
I grant permission to Grind Basketball to photograph, record, and/or videotape my child during training sessions, camps, games, or related activities.
I understand and agree that these images, video recordings, and audio recordings may be used by Grind Basketball for promotional, marketing, educational, and informational purposes, including but not limited to use on websites, social media platforms, flyers, advertisements, and other digital or print materials.
I acknowledge that:
No compensation will be provided for the use of my child’s likeness.
All media content becomes the property of Grind Basketball.
Such materials may be edited, reproduced, published, or distributed without prior notice or approval.
I release and hold harmless Grind Basketball and its representatives from any claims related to the use of such media, including claims of invasion of privacy or misrepresentation.
