Permission & Participation
“I give permission for my child to participate in all basketball training activities organized by Grind Basketball during the selected camp, clinic, or training session.”
Assumption of Risk & Liability
“I understand that participation in sports involves inherent risks, including but not limited to physical injury, paralysis, or death, as well as risks associated with travel to and from training sessions, games, or events. I voluntarily assume all such risks and accept full responsibility for any injury, loss, or damage that may occur. I agree to release and hold harmless Grind Basketball, its owners, staff, coaches, volunteers, contractors, and any associated facilities or partners from any and all claims, liabilities, or demands arising from my child’s participation, except where prohibited by law.”
Minor Pickup & Safety Policy
“All participants under the age of 14 must remain inside the gym at the conclusion of each training session and may not leave the facility without being picked up by a parent or authorized guardian. Coaches and staff are not responsible for participants who leave unsupervised. Parents/guardians are expected to arrive on time; failure to comply may result in suspension from future sessions.”
Media Release
“I grant permission to Grind Basketball to photograph, record, and/or videotape my child during training sessions, camps, games, or related activities. I understand and agree that these images, video recordings, and audio recordings may be used by Grind Basketball for promotional, marketing, educational, and informational purposes, including use on websites, social media platforms, flyers, advertisements, and other digital or print materials. No compensation will be provided for the use of my child’s likeness, all media content becomes the property of Grind Basketball, and such materials may be edited, reproduced, published, or distributed without prior notice or approval. I release and hold harmless Grind Basketball and its representatives from any claims related to the use of such media, including claims of invasion of privacy or misrepresentation, and waive any right to inspect or approve the finished product or its use.”
Governing Law:
This Agreement shall be governed by and interpreted in accordance with the laws of the Territory of Guam, without regard to its conflict of law principles.
