Authorization Form 

We (I) authorize an adult, in whose care the minor has been entrusted, to consent to any x-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment, and hosptial care, to be rendered to the minor under the general or specific supervision and on the advice of any physician or dentist licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital, wether such diagnosis or treatment is rendered at the office of said physician of at said hospital. The undersigned does hereby give permision for our (my) child to ride in any vehicle designated by the adult in whose care the minor has been entrusted while attending the Chambersburg Project. 

I (we) the parent(s) or legal guardian(s) of this participant assume all responsibility and agree to hold harmless and indemnify Chambersburg Project Inc. as the result of neglligent, willful, or intentional acts of my (our) son or daughter that may result in claims and demands for personal injury, sickness and death, as well as property damage and expenses. I (we) authorize Chambersburg Project, Inc. to furnish transportation for my (our) son or daughter. 

I herby grant Chambersburg Project, Inc. permission to interview me/my child and or to use my photograph(s)/video in any and all publications and in any and all other media wheather now known or hereafter existing, controlled by Chambersburg Project, Inc. in perpetuity, and for other use by Chambersburg Project, Inc. I will make no monetary or other claim against Chambersburg Project Inc. for the use of interviews or the photograh(s)/video. 

I also acknoledge that all of the above medical, transportation and photo release clauses are granted and apply to the Falling Spring Presyterian Church and their youth leaders in regards to my childs participation in Chambersburg Project July 6-11, 2025. 

Covenants & Expectations

Parents and youth please read the following covenants and expectations and sign. By signing you are agreeing to honor the covenants and expectations set forth by Chambersburg Project, Inc. 

1. Honor God with your words and actions! Seek to serve Him in everthing (1 Cor. 10:31)

2. All youth/adults must wear a wristband at all times- it's your meal ticket and your admittance ticket to the pool on Wednesday! 

3. Understand that you are a guest at the host facility and your worksite. Stay away from off limit areas. No boys' in the girls sleeping areas, and no girls in the boys' sleeping areas. Keep all areas (including yourself) neat and clean.

4. Lights out at 11pm! No exceptions. Respect others and be considerate. 

5. No sexual harassment of any kind will be tolerated. Any form of harassment should be immediately reported to the leadership.

6. Be on time and present for all activities.

7. Do not bring illegal substances, such as tobacco, alcohol, or other drugs firearms, knives, lighters, matches, etc.

8. You must stay with your crew at the worksite, and remain with them the entire week. You may be required to switch crews, but may only do so at the discretion of the leadership. You must also stay with the group at all other times and are not permitted to leave, unless you receive permission from the leadership and prior arrangments have been made. 

9. Report any damage or hazardous conditions to a leader.

10. Bring appropriate work and swimwear. No tanktops for working on jobsites due to safety. 

11. All electronic devices, such as cellphones will be brought at your own risk. We ask that all electronic devices are turned off at 11pm to respect others. 

12. Closed toe shoes must be worn while the participant is on the jobsite.

*Violations of these covenants may result in your youth being sent home.

Cost: $225/person.

Refund Policy: If you can no longer attend Chambersburg Project, please let us know on or before June 1, and you will receive a full refund.
