Please use this form to signup for either the upcoming in-person Mideast Regional training or the virtual session that will be held. Note: The virtual session will not include the required in-depth practicum portion of the day; for that, a follow up discussion and session will need to be scheduled.

I understand that this session will be conducted in-person at the SVDP Cincinnati location (1125 Bank Street, Cincinnati, OH 45214) from 9am to 3pm. All materials will be provided to me as a participant, and lunch is included. Free parking is available onsite in our gated lot.*

The Ozanam Center at SVDP Cincinnati is an immersive retreat center complete with a residence wing (shelter style bunk beds, showers, bathrooms) and full use kitchen. If you are coming from a greater distance and would like overnight accommodations at no cost, you are welcome to stay in the center. Alternatively, you are also welcome to more private lodging in the Cincinnati area at one of the hotels. *Please note lodging at the Ozanam Center is limited and will be confirmed with you after selection.*

I do not need overnight accommodations, thank you.I will find overnight accommodations at a nearby hotel.I would like to stay overnight in the Ozanam Center.I am unsure at this time, please check in with me two weeks prior to the training.
