The staff and volunteers of Brooks Evangelical Free Church will do their best to provide your child with a safe and caring environment when entrusted to our care. However, not all accidents can be prevented, therefore we ask you to sign the following waiver, which includes a medical release and photos/videos taken release.
Brooks Evangelical Free Church and its staff, board and volunteers will not be held responsible in the event of any accidents or injuries. Your children will be having fun, jumping, singing and playing games. We also, take pictures and videos of all the children having fun. In signing this waiver, you give permission for pictures to be taken of your child while they are participating in VBS activities for the use of Brooks Evangelical Free Church Children’s Ministries and promotions of those ministries.
Medical Consent
Information received is confidential and is being gathered for the purpose of serving your child better while in the care of Brooks Evangelical Free Church. Any medical information collected serves to authorize Brooks Evangelical Free Church, and its staff and volunteers, to obtain medical assistance in emergencies.
Every effort will be made to contact the parents or guardians prior to needed medical treatment. In the event that the parents or guardians are not accessible, I/We the parents or guardians authorize the ministry staff of Brooks Evangelical Free Church to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment, or procedures for the child named above.
I, named below, undertake and agree to indemnify and hold blameless the ministry staff, Brooks Evangelical Free Church, its pastors and Board of Elders from and against any loss, damage, or injury suffered by the participant as a result of being part of the activities of Brooks Evangelical Free Church, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in, or travelling to, events of Brooks Evangelical Free Church.
