CAMP RELEASE AND WAIVER OF LIABILITY AND CONSENT

In consideration of being permitted to participate in the 2025 E-Line Softball Summer Camp, I (parent/guardian) hereby assume the risks of personal injury that may result from program activities. I am aware of the potential for injury while participating in the sport of softball. The undersigned hereby acknowledges that participation in this camp and related activities involves an inherent risk of physical injury, and the undersigned, on behalf of the registrant, hereby assumes all such risk and does hereby release and forever discharge the camp and all employees and agents thereof from any and all liability of whatever kind of nature, arising from and by reason of any and all known and unknown injuries, damage to property, and the consequences thereof, resulting from the registrant’s participation in or involvement with this camp, including and failure of equipment or defect in the premises.

I warrant and represent, to the best of my knowledge and belief that the participant is healthy and able to participate in the camp, and I agree to notify camp administrators of any allergies or other physical, mental or emotional condition that might limit the participant’s ability to safely participate in the camp activities.

I give permission to E-Line Softball Training Softball Camps, its trainers or other staff members and agents to provide such emergency care and treatment to the participant, as in their judgment may be deemed necessary or advisable in the event that the participant should require emergency care while participating in the camp. I agree to assume the costs of such emergency care and treatment, if any such costs are incurred.

MEDIA CONSENT:

I give my consent and permission for the taking of photographs and/or video of my child during the camp and waive and/or assign any and all rights (including copyright) in such media to E-Line Softball Training. E-Line Softball Training, as the sole owner of such media, shall have exclusive rights to control and determine the use, display, performance, reproduction and dissemination of any such photographs and/or videos.

 
I hereby release and agree to hold E-Line Softball Training harmless from, and waive on behalf of myself, my daughter, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to my child and/or property that may be caused by any act, or failure to act of the facility, or that may otherwise arise in any way in connection with any services received from E-Line Softball Training. I understand that this release discharges E-Line Softball Training from any liability or claim that I, my daughter, or any personal representatives may have against the facility with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from E-Line Softball Training. This liability waiver and release extends to the facility together with all owners, partners, and employees.
