  • Northwest Rock, Inc. (refered to as NW Rock hereafter) has compiled a list of hazards, rules, regulations, and applicable procedures to ensure compliance with MSHA Site-Specific Hazard Awareness Training and to protect the safety of all individuals on our property. All contractors, vendors, visitors, and others must read and understand this information prior to entering any of our facilities. If you have any questions or need clarification, you are responsible for seeking assistance from a NW Rock representative. Failure to comply with any of these safety rules, policies, or procedures may result in immediate and permanent dismissal from all NW Rock properties. 

    **Please note, training is not required for any person who is accompanied at all times by an experienced miner familiar with hazards specific to the mine site (§46.11 (f)).**

    Instructions: Completion of all sections is required and must be submitted to NW Rock (electronically or by hand) PRIOR to authorized entry. Electronic submissions are automatically sent to NW Rock. If you prefer to complete a hard copy, please visit the main office or scale shack on-site, and we will be happy to provide one for you.

    NOTICE:

    1. Site Personnel and Visitors: A current certification is required for all personnel entering a NW Rock site or facility (i.e., office or staff personnel, scientific or delivery workers, construction workers or employees of independant contractors, vendors, visitors, maintenance or service workers, and all other customers (including commercial drivers, the general public, etc.)). The certificate is located at the end of this document.
    2. Current Certificate: If you have a valid certification on file with NW Rock—issued within the past two (2) years or since the most recent update to site hazards—you are not required to submit a new certification.
    3. Non-transferable: Certifications are site-specific and do not transfer between locations. A current certification is required for all entering each NW Rock site or facility.

  • SAFETY Rules -

    Decker Creek Pit
    1. Contractors, Customers, Visitors, and Vendors: Outside contractors, vendors and visitors will abide by all NW Rock  safety rules, Occupational Safety & Health Administration (OSHA) regulations, MSHA regulations and safety training requirements as mandated by NW Rock policy, OSHA and MSHA (specifically those for Metal & Non-Metal mines located in 30 CFR, parts 48, 56, & 58).
    2. Personal protective Equipment (PPE): Required PPE, which may include hard-hats, safety glasses and steel toed footwear, shall be worn by any personnel entering NW Rock property (except office areas).
    3. Hearing Protection: Hearing protection shall be worn in areas so designated by posted signs.
    4. Fall Protection: When working or walking in elevated areas that are not protected by handrails, a safety harness with lifeline must be used for fall protection.
    5. Traffic Safety: All posted traffic rules and regulations are to be followed at all times.  Speed limit is 10 miles per hour on NW Rock property, unless otherwise posted.
    6. Authorized Access Limitations: When entering NW Rock property, travel should be limited to the specific area where your services are required.  Travel to other areas without authorization is not allowed.
    7. Parking and Vehicle Right-of-Way: Vehicles are not to be parked in the working area of any heavy equipment or mobile equipment.  NW Rock’s equipment has the right-of-way at all times.
    8. Exiting Vehicles: All persons entering NW Rock property in a customer or contractor truck must stay inside the cab of the vehicle, and/or must follow the directions of the loader/excavator operator while being loaded.  If exiting the vehicle is necessary, all NW Rock safety rules, including wearing a hard hat, are to be followed.
    9. Equipment Standards: Any equipment used on NW Rock property shall meet the minimum standards of MSHA/OSHA.
    10. Equipment Use Authorization: No NW Rock equipment shall be used for contractor work without permission from the NW Rock supervisor in charge of such equipment.  NW Rock equipment shall be operated by NW Rock personnel only, unless authorized by the site manager.
    11. Mobile Equipment Safety: Mobile equipment shall not be left unattended unless placed in park and the parking brake, if provided, is set. When parked on a grade, the wheels or tracks shall either be chocked or turned into a bank or rib.
    12. Mobile Equipment Operation: Before starting or moving mobile equipment, operators shall sound a warning that is clearly audible above the surrounding noise level to alert nearby personnel (i.e. single horn honk).
    13. Drug and Alcohol Policies: All contractors, vendors, visitors and their employees are expected to comply with NoW Rock’s drug and alcohol testing policy.  Refusal or failure to participate, or a positive test may result in immediate and/or permanent dismissal from all NW Rock properties.  Alcohol is strictly prohibited from all sites.
    14. Smoking: All "no smoking" signs must be strictly obeyed.  Company policy prohibits smoking in any office space, as well as posted areas around fuel tanks and explosives.
    15. Weapons: No weapons or firearms are permitted on NW Rock property.
    16. Chemicals On-site: Chemicals are in use on NW Rock properties.  Before working with or around any of these potentially hazardous materials ask for information and/or Material Safety Data Sheets (SDS) about them and the dangers they pose. No contractor, vendor or visitor may bring any chemicals onto NW Rock. property without first presenting a current SDS for each one to the Safety Officer or Pit Foreman and receiving authorization.
    17. Pressurized Bottles: All oxygen, acetylene and other pressurized gas bottles belonging to contractors, vendors or visitors shall be secured at all times.  When not in use, bottles should be capped.

  • LOCKOUT/TAGOUT Policy -

  • When inspecting/repairing any piece of moving machinery on NW Rock property, contractors, vendors, and visitors shall abide by MSHA regulations on lockout/tagout including:

    1. Guards: All guards shall remain in place until the machine has stopped.
    2. Lock Out & Tag: Once stopped, the power shall be cut off at the main power supply, and locked out and tagged with the name of the person performing the work.
    3. Test: Equipment shall be tested to insure that power supply has been disconnected and all forms of residual energy have been relieved.
    4. Repair: Once lockout is insured, guards may be removed and repair work begun.
    5. Re-energize: All machine guards shall be secured in place before removing padlocks and re-energizing machine, except as otherwise specifically authorized by plant manager or his authorized representative for testing purposes only.
    6. Lock Removal: Safety locks should be removed only by the persons who installed them and whose name is on the tag.

  • CERTIFICATE/RECORD:

  • In accordance with §46.12 of the Mine Safety Act, production-operators have the primary responsibility to ensure that site-specific awareness training has been given. All mining operations have hazardous areas, and this information must be communicated to all those who enter the site for “frequent and extended periods”. Warning signs are located throughout the site to notify of these hazards.

    **False certification is punishable under section 110 (a) and (f) of the Federal Mine Safety and Health Act.**
