COUNTER PLEA

I have received a copy of the criminal complaint and I understand the charges against me. I previously entered a plea of not guilty. I now hereby plead to the charge(s) amended charge(s) as alleged below:  

I HEARBY PLEAD   GUILTY   NO CONTEST TO THE ABOVE CHARG(S). I understand that I amwaiving my right to enter my plea in open Court before the Judge and agree that formal judgment may be entered atany time following this PLEA AND WAIVER. I further understand I have the constitutional rights listed below, and I am now waiving these rights:  

1. I have the right to a speedy trial.
2. I have the right to make the State prove the charges against me beyond a reasonable doubt.
3. I have the right to confront and cross-examine any witnesses that are called.
4. I have the right to use the subpoena power of the Court to bring in witnesses to testify in my own favor.
5. I have the right to remain silent. I do not have to make a statement at any time, and at the time of trial, I do not have to testify. If I choose not to testify, that fact will not be held against me.
6. I have the right to an attorney, and if it is a serious misdemeanor for which there is a likelihood that I will be sent to jail, then I have the right to a court-appointed attorney to represent me if I cannot afford to hire my own.
7. I understand that the maximum penalty for a misdemeanor is up to six (6) months in the county jail and up to a $1,000.00 fine.
