Volunteer Attestation Form

As a Volunteer of Jarvis District Christian School (the “Corporation”), I do hereby agree to the following statements:

1. I have reviewed the Corporation’s by-laws, Constitution, Statements of Faith, Policies, and the Presbyterian Church in America’s “Report of the Ad Interim Committee on Human Sexuality,” and I acknowledge that to remain in harmony with the Corporation and its Members I shall conduct my actions in accordance with all such documents and when participating or attending the Corporation, its events or functions, I shall endeavor to live according to the beliefs expressed therein.
2. In particular, I shall ensure that my actions, both in word and deed, are consistent with the twelve statements on human sexuality indicated in the “Report of the Ad Interim Committee on Human Sexuality,” and summarized as follows:
   1. Marriage is to be between one man and one woman, and that sexual intimacy is reserved for marriage, in accordance with the institution of marriage created and ordained by God.
   2. Human beings are created in the image of God and created as only male or female and we should commit ourselves to live in accordance with God’s design for us in accordance with the biological sex He has created for us.
   3. That all of man is inherently corrupted by the original sin of our first Parent / Guardians, Adam and Eve, and that we must repent of our sins to God and endeavor to walk in accordance with His commandments. That desire of same-sex attraction is not morally neutral, but like any other sin, is a result of our sinful nature which we should fight against and repent from.
   4. That Christians may struggle with same-sex attraction, gender identity issues, immoral sexual desires outside of marriage, and other sexual sins, but that despite such struggles, we are called to a life of repentance. The struggles of such a penitent sinner should be seen as a courageous example of faithfulness, and as a Christian community we should encourage and uphold such persons that struggle and repent
      of such sins.
3. I understand that my involvement with the Corporation, shall be conditional upon my compliance with the aforementioned documents and beliefs and that the Board shall have the right to terminate or prevent my involvement with the Corporation if they determine in good faith that I have failed to do so.
4. I shall endeavor in every way possible to commit myself to following and adhering to the Biblical teachings espoused by the Corporation.
5. I am an active member and/or participant of a Bible-believing Church and that my involvement with the Corporation is conditional upon my continuation of my active Church attendance and/or membership.
6. I have had an opportunity to review the Corporation’s policies and shall comply with them as long as I am involved with the Corporation.
7. I acknowledge that the Corporation’s programs, curriculum, activities, materials, classroom discussions, and all elements associated with the Corporation shall reflect the beliefs reflected in this attestation.
8. I acknowledge that the Board of the Corporation shall have the authority to determine when my actions are inconsistent with the statements I have agreed to herein, and that such a determination may result in my removal as employee, the inability for my children to attend the Corporation, or my inability to become a volunteer of the Corporation. I agree to abide by the Board’s decision which shall be made in accordance with the Corporation’s by-laws, Constitution, Policies, and Statements of Faith.

By signing this attestation, I confirm that I have read all the statements indicated herein and that I agree with them and was not compelled to sign this attestation by any entity or person, but rather that I have signed this attestation of my own free will.
