INSTRUCTIONS

We suggest you download and review a Sample Application Form so you may collect the necessary information BEFORE you begin completing the online form. Download Sample Application Form

Complete all sections and upload the required documents. You may Save and Return at any time and finish your application later, but to do so you must provide an email address in the application form. Locate the Save & Continue Later button at the end of the form. When saving, ignore the prompt to create a JotForm account - instead choose Skip Create an Account. The form will then email you a link, allowing you to continue.
- Uploading Documents: PDF is preferred file format (optional .jpg, .docx)
- Official Transcripts: Please have the college/university email the Official Transcripts directly to the Regional Office to ebeer@okdisciples.org.
After completing the personal information, education and work history, complete the Personal Disclosure statement and add your signature. Then indicate the Type of Ministry you are seeking (i.e., Commissioning, Ordination). If you are unsure, do not answer this section until you discuss it with the Regional Minister.

You may Preview the document before submitting to review your answers. Upon choosing Submit you will be emailed a copy of the completed application, and it is sent to the Regional Office.

Next Step: Once the application and all required documents have been received, the Regional Minister will review them and contact you to schedule an appointment to discuss your application. Please note, your file must be complete with all required documents before the Regional Minister may present it to the Oklahoma Commission on Clergy for consideration.

- Signed Ministerial Code of Ethics (download)
- 3 letters of reference from: (1) an elder of the congregation where you hold membership; (2) a Disciples or UCC minister; and (3) a lay member of a Disciples or UCC congregation who may speak to your gifts for ministry.

1. Answer the Personal Disclosure statements below and sign.
2. Indicate the Type of Application (i.e., Commissioning, Ordination).
3. Submit your application for consideration by the Commission on Clergy.
Once all documents have been received, the Regional Minister will contact you to schedule an appointment to discuss the Type of Application (shown below) and to grant you permission to complete a Ministerial Profile.

(3) Authorization to perform ministry on behalf of the Christian Church (Disciples of Christ) as an ordained minister by completing seminary.

(4) A minister ordained by another denomination who wishes to transfer her/his credentials to the Christian Church (Disciples of Christ).
