PERTINENT REGULATIONS:
An official Health Certificate (CVI) must accompany the Import Permit Application, including all testing requirement results. Specific requirements may exist for individual species and should be obtained by email at . Animals are subject to inspection by VIDA personnel upon arrival.

This import permit is not transferrable or assignable and expires within 30 days of date of issuance.

Acceptance of this import permit is contingent upon the Consignee's agreement to comply with retention permit #
