Medical Release

I, {primaryParentguardian}, the parent or legal guardian of {studentName}, give permission to Enlightened Minds and vendors for any and all medical attention to be administered to my child in the event of an emergency, when the need for treatment is clear, and when all efforts have been made to contact me. By signing below I also assume the financial responsibility of payment for any and all expenses incurred for treatment.

Waiver of Liabiity 

 I, {primaryParentguardian}, parent or legal guardian of {studentName} give my full permission for my child to participate in classes and activities at Enlightened Minds' Vendor Campus. In allowing my child to participate in classes and activities at Enlightened Minds' Vendor Campus, I fully and completely release, discharge, and hold harmless Enlightened Minds, and its directors, trustees, officers, employees, agents, insurers, independent contractors, vendors, and volunteers (collectively its employees and agents) from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury, death, or property damage, arising out of my child's participation in classes and activities at Enlightened Minds' Vendor Campus.

I acknowledge and recognize there are inherent risks involved in such activities and I assume the risk of any injury sustained while at Enlightened Minds' Vendor Campus. I agree to indemnify, defend and hold harmless Enlightened Minds and its employees and agents from any and all claims arising out of my child's participation in any class or activity at Enlightened Minds' Vendor Campus.

Media Authorization and Release

Enlightened Minds and its vendors request permission to photograph and/or record your child while participating in classes and/or activities and for the resulting images and/or recordings to be used for internal newsletters, social media marketing, and/or training purposes.

By signing below, I hereby release and hold harmless Enlightened Minds and its employees and agents from any all claims arising from the use of photographs and/or recordings.

Technology Policy

 I, {primaryParentguardian}, parent or legal guardian of {studentName} understand that cellphones, tablets, smartwatches, laptops, and similar electronic devices are not allowed to be used for entertainment purposes while my child is on campus. Cellphones and smartwatches should only be used to communicate with parent or legal guardian and should be placed on silent during my child's time at the learning center to foster a focused and distraction-free learning envrionment and to ensure student safety and well-being for all students on campus. Tablets, laptops, and similar electronic devices should only be brought and used for educational purposes when my child will be doing academic work in a class on campus.

I understand that if my child brings any of these electronic devices to campus, I am responsible to share the expectation of this policy with them. I understand that if my child has a hard time following this policy, Enlightened Minds and its vendors may ask that my child not bring the device or another alternative be followed.

Communication: If I have any questions about the campus I will reach out to Enlightened Minds via email (info@enlightenedminds.net) or by phone (559-569-1375). If I have any questions relating to a specific class, I will reach out directly to the Vendor offering the class.

Drop off/Pick up/Property: I understand I have the option to drop off or stay on campus when my child attends class(es). I understand it is my responsibilty to sign my child in for their first class and sign my child out when picking up. If I choose to drop off, I will drop off and pick up on time. If I choose to stay on campus, I understand it is my responsibility to clean up after myself and my child(ren) and to respect and take care of the property (i.e. no climbing fences, digging, picking flowers or leaves off bushes or trees, etc). I also understand smoking is not permitted anywhere on campus.

Water, Snacks, and Lunch: If my child has two or more classes or will attend Campus Lunch, I will make sure they have a water bottle and snacks or lunch.

Holidays/Emergency/Class Cancellations: I am aware that some of the class days may fall on a holiday and that some class days may close due to holidays. In the event of closure due to emergency or other unforeseen circumstances, I am aware that it is up to each individual Vendor to decide if they will provide a make-up day.

Behavior & Conduct: I will do my best, within my abilities as the parent/guardian, to encourage my child to be prepared for class and ready to participate with a positive attitude. I will instruct my child to show respect for Enlightened Minds instructors, vendors, and other students. I will share with them the expectation that they follow the rules, which includes instructing them to remain in their class(es) and not wander off into other areas of campus. I understand that if there are ongoing behavioral issues or other concerns regarding my child's participation and/or behavior on campus, Enlightened Minds and/or Vendors may come to the conclusion that Vendor Campus is not a good fit for my child, and that they may be dismissed from further participation. 

Weekly and Monthly Class Fee: I understand that I am responsible for my child's class fees, which must be received by each Vendor before the first class and that they may not be allowed to attend class(es) if the payment has not been received and/or confirmed.

Charter School Students: I understand that it is my responsibility to submit the Vendor service order for the class fee in a timely manner. This usually requires that I submit the order two (2) weeks prior to the start of class, in order for the Vendor to receive my approved certificate before class begins. 

I also understand that it is my responsibility to be informed of my charter school's specific guidelines regarding Vendor service orders. I understand that if I request services from my charter for use at Enlightened Minds' Vendor Campus, and I do not utilize the services, that my charter school may not issue me a refund for unused services, and that it is not the financial responsibility of Enlightened Minds or any Vendor to reimburse me for unused services.

Property Damage: I understand that if my child is found to be responsible for any damage to the property, I may be financially responsible for the damages.

Refunds/Credits: I understand that it is my responsibility to be informed about specific Vendor policy on refunds or credits for missed days due to illness, vacation, or any other circumstance. I understand Enlightened Minds does not provide refunds or credits for any reason for their class offerings on Vendor Campus days.
