  • LEARNING CENTERS ATTENDANCE AGREEMENT

  • I understand the following:

    • In order to maintain services, my child must attend the Learning Center as authorized and scheduled.

     

    • I agree that I will follow the schedule and days I have selected my child will be attending. If I wish to make a schedule change, I am required to give the Learning Center a 30 day notice, and the Learning Center Assistant Director must approve before a change can occur.

     

    • I am allowed 2 weeks per calendar year of vacation time, which must be used in 1 week consecutive increments. Outside of these 2 weeks, I will be charged for a week of service whether my child attends or not. This also includes holidays, in-service days, and/or calamity days when the Learning Center is closed.

     

    • I agree to contact the Learning Center before 9:00 AM, any day the following occurs (unless a situation was discussed with the Assistant Director and approved beforehand):
      • My child will be absent.
      • My child was diagnosed with a communicable disease (chicken pox, influenza, etc and may have exposed others at the Center.
      • My work schedule changed (called in early, working late)*

     

    • I understand my child must arrive to the Learning Center by 9:00 AM to attend for the day.

     

    • I will ensure my child is accurately clocked in and out every day through ProCare or KinderConnect tablet (ODJFS clients only):
      •  As an ODJFS client, either I or my appointed adult designee must clock in and out on the above listed devices, or the child will not be permitted to stay.
      • As an ODJFS client, I will clock my child in upon entering the building for drop off times and clock out after children have been picked up from their classrooms. I will instruct my authorized emergency contacts of the procedures for picking up or dropping off my child.
      • As an ODJFS client who may need to approve a pending transaction in the KinderConnect system, I understand it is solely my responsibility to approve transactions ASAP. I further understand that I will be billed the JFS reimbursement rate for any days my child has attended but I failed to approve.

     

    • I understand failure to communicate with the Center for 10 business days may result in termination of my child care services. 
    • JRC will not be responsible for personal belongings left in the Center two (2) weeks beyond the last day of attendance. 
  • This agreement applies to all JRC Center clients whether paying privately or receiving JFS subsidy rates.
