    ☐ I understand Elite Notary Signing is a notary service provider and does not offer legal advice or prepare legal documents.

    ☐ I understand all documents must be complete and unsigned prior to the appointment.

    ☐ I understand signers must be mentally aware, willing, and present with valid identification at the time of notarization.

    ☐ I understand witnesses, if required, must meet eligibility requirements. If Elite Notary Signing provides witnesses, availability is not guaranteed.

    ☐ I understand appointments outside standard business hours or requiring expedited service may include additional fees.

    ☐ I understand that a $25 deposit is required to confirm my appointment. This deposit is applied toward the total service fee and is non-refundable. The remaining balance is due at the onset of service, and payment instructions will be provided once my request is reviewed and approved by Elite Notary Signing.
